Dear Mr. Keppler:
On August 18, 1982, Mr. T. D. Geib of Public Service Company of Indiana, Inc. (PSI) notified your office of a potentially-reportable item as required by 10 CFR 50.55(e). This problem involves embedded plates in the primary shield wall having deformed wire anchors missing or the welds between the plate and anchor being cracked. These plates ,
had been installed prior to the cessation of safety related work in
,, August, 1979.
For the subject embedded plates, alternative support mechanisms were utilized which resulted in acceptable structural integrity without taking credit for any of the deformed wire anchors in question.
Procedure reviews have been made to establish a cause for missing or broken deformed wire anchors. Tests have been performed in order to establish the failure mode of the cracked welds. PSI ~
evaluation of the results of these reviews and tests is continuing.
This letter is intended to fulfill the requirements of an interim report as defined in 10 CFR 50.55(e). A further report will be filed on or before May 11, 1983. If you have any questions regarding this matter, please contact. me at your convenience.
