Dear Mr. Keppler:
This letter represents our final report pu rsuant to 10CFR50.55(e) on the significant deficiency concerning the Cutler-Hammer motor control centers.
This report is being submitted to summarize all of our previous correspondence and to provide clarification of the corrective action taken. Reports filed on this subject were dated June 1980 through April 1982.
This repect includes a description of the deficiency, an analysis of the safety implication, and the corrective action taken.
lescription of Deficiency Through our procurement specification, Cutler-Hammer was contracted by The Cleveland Electric Illuminating Company to manufactere Clasr lE and Non-Class IE motor control centers for the Perry Nuclear Power Plant. To permit interchangeability of buckets between safety-and nonsafety-related motor l
control centers, all starters, branch feeders, and other units were designated to be built to Class 1E requirements. While performing minor repair work on starter units installed at the Perry Nuclear Power Plant, a representative l
f rom Cutler-Hammer discovered several of the units contained control wire not qualified to ;EEE requirements and that several of the units contained either unqualified terminal blocks or improper saddle clamps.
Cutler-Hammer sub-sequent?y inspected all 1,943 starter units on site and identified all units l
containing either of these deficiencies in May of 1980.
A subsecuent inspection of the identified defective units on site by PNPP resulted in a revision to the number of defective units but also identified that four (4) splice plate carriage bolts had broken during installation on the bus.
Analysis of Safety Implication The safety ~related mo+or control centers are necessary for the saf. operation and shutdown of the plant.

Corrective Action Taken In order to correct the deficiencies, the following steps were taken:
1.
The safety-related starters originally identified by Cutler-Hammer with unqualified wire were returned to the vendor for rework with prequalified wire. The units were returned to the site and have now been installed.
2.
All of the originally identified unqualified terminal blocks used in safety-related applications were replaced by Cutler-Hammer.
Because of the change in quantities identified as defective by Cutler-Hammer, a sample of 200 starter units were examined February 1981,. by PNPP Inspectors. No defects were noted during this inspection.
However, in September another unqualified terminal block was discovered in a unit which was not part of the 200 inspected by site, nor was it originally identified by Cutler-Hammer. As a result of thia finding, a 100% inspection of the remaining MCCs was performed by PNPP personnel. All additional-deficiencies were identified on site nonconformance reports.
The original site position to maintain interchangeability between safety-related and nonsafety-related units has been modifled and will not be allowed during field installation or plant operation. The procurement specification was revised to reflect this decision. In order to assure this, each bucket will be identified by color coded lamicoid tag to

indi ate safety designation.
Subsequently, those units which are now classified as nonsafety-related will not require modification as they are acceptable commercial grade items which would not require IEEE _ qualification.
3.
All splice plate carriage bolts were replaced.
An extensive test program was developed with Cutler-Hammer to ensure the adequacy of the repJacement bolts. Replacement bolts were uniquely stamped at receipt on site and the replacement documented.
