On December 8,1982, Florida Power & Light Company notified NRC Region 11 of a potential 10 CFR 50.55(e) condition existing at the site involving incorrect switch development. Pursuant to the requirements of 10 CFR 50.55(e), a final report is attached.

I.

Summary a) The atmospheric steam dump valve motor operated isolation valves I-MV-08-14, I-MV-08-15, I-MV-08-16 and I-MV-08-17 are controlled from the control room and from the local control stations.

These valves are normally locked open and are closed only upon an atmospheric steam dump valve-failure to close.

The key lock switches are utilized to lock these valves open.

b) The atmospheric steam valve motor operated valves I-MV-08-18A

  • I-MV-08-18B,,I-MV-08-19A and I-MV-19B are normally, closed during plant operation.

These valves are operated, i.e. open-ed or closed, during reactor hot standby, from the control switches in the control room or from the hot shutdown panel outside the control room in a manual or automatic valve con-trol mode.

Description Construction Quality Control upon examination of the control switches CS-1621-1, CS-1622-1, CS-1623-1 and CS-1624-1 for the atmospheric steam dump valve isolation valves and of the selector switches SS-1625-1, SS-1626-1, SS-1627-1 and SF-1628-1 for the atmospheric steam dump valves, has established that the system did not work because the switch contact developments were not in confomance with the Specification FLO-2998.144 Sheet EL-17 Equipment List nor with the Control Wiring Diagrams 2998-B-327 sheets 1621, 1622, 1623, 1624, 1625, 1626, 1627, and 1628.

This deficiency was discovered during normal routina pre-operational testing.

III. Corrective Action The control switches listed above were replaced with an apprc3-riate new set -of switches having correct switch contact develo3 -

ments.

IV.

Safety Implications We have evaluated this concern and determined that a deficiency in component construction existed which if left uncorrected would have affected safe reactor shutdown, by rendering the atmospheric steam dump system inoperative from the control room.

These valves could have been locally manually operated, however, this would violate Branch Technical Position 5-1 which states that all con-trols be operated from inside the control room.

Conclusion As a result of the above, we deem this' item to be reportable under 10CFR50. 55 (e).

With the corrective action indicated, this response closes out this item regarding reportability requirements.

All pertinent documentation will be maintained at the site.
