4 NIAGARA M0 HAWK POWER CORPORATION NINE MILE POINT UNIT 2 DOCKET N0. 50-410 RESPONSE TO NOTICE OF VIOLATION ATTACHED TO NRC INSPECTION REPORT N0. 50-410/82-12 The apparent violation was identified as follows:

10CFR50, Appendix B, Criterion V, states in part, " Activities affecting quality shall be prescribed by document instructions, procedures.... and shall be accomplished in accordance with these instructions, procedures or drawings".
ITT Grinnell Industrial Piping, Inc., ASME Quality Assurance Manual, QCF-4.1, Process Control, Revision 7, states in part that the field engineer shall submit the field planner to the Quality Control Documentation Supervisor for review and approval of adequate instructions and inclusion of all required procedures for work to be performed and adherence to specification requirements.
Contrary to the above, Field Planner SB-400 for ASME Boiler and Pressure Vessel Code, Section 3, Class 3, Service Water Piping was not submitted to the Quality Control Documentation Supervisor for review and approval. As a result, the field planner had been released by Engineering for j
fabrication and installation with welding control and material requirements not correctly specified.
This is a Severity Level IV Violation (Supplement II).
The following is submitted in response to this item of nonconformance:
Field Planner SB-400 has been cancelled. As stated in the Nuclear Regulatory Conmissio-inspection Report 82-12, ITT Grinnell issued a stop work order on all small bore piping in order to determine if any other small bore planner packages were issued with the incorrect category designation.
In addition, ITT Grinnell reviewed 679 Category II large bore piping planner packages to ensure the correct category designation.
ITT Grinnell's review of all other small bore planner packages and the 679 Category II large bore piping planner packages indicated that all had the correct QA category designations.
ITT Grinnell is modifying General Engineering Procedure (GEP) 40 to incorporate a checklist to ensure that the correct QA category designation is j
specified. This checklist will be maintained in the ITT Grinnell Engineering office and attached to the original planner.
In response to Corrective Action Report 539, ITT Grinnell has implemented the checki:. in their review cycle for issuing all planners. The ITT Grinnell General tngineering Procedure (GEP) 40 will be issued by January 31, 1983.
