APPENDIX A Stone and Webster Engineering Corporation Docket No. 99900509/82-02 NOTICE OF NONCONFORMANCE Based or, the results of an NRC inspection conducted.on August 16-20 and August 30-September 3,1982, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR Part 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, cr drawings.
Instructions, procedures, or drawings shall. include appropriate quantitative-or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
NUPIPE Stress Analysis and Supports Evaluation Procedure, dated January 15, 1980, states in part:
"8.2 The completed package shall be reviewed and approved by the Project, Engineering Mechanics, and EA. This review and approval shall be documented on Form Z.
The Project Engineer's signature signifies that the Project has confirmed that the package identifies the acceptability of the pipe stress analysis, pipe supports (including baseplates), ecuip-ment nozzles, containment penetrations, and equipment anc structural supports. The Engineering Mechanics signature signifies that the Principal PSAS Engineer has reviewed and is in agreement with the method of analysis and the results.
The EA signature signifies that EA has audited the package or portions of packages for which they are responsible and all EA findings (if any), have been resolved.
"8.3 Completed packages shall become a pemanent part of plant records...."
Contrary to the above, activities affecting quality (preparation and control of pipe stress analysis and support calculation packages for Beaver Valley 1) were not accomplished in accordance with the prescribed NVPIPE Stress Analysis and Supports Evaluation Procedure.

The-Engineering Assurance (EA) " Action Items Tracking Log - EA Audits Projects" listed 17 documents that identify unsatisfactory conditions or questions from EA audits of Beaver Valley 1. stress calculation packages that have not~been resolved.
