FINAL REPORT --DER-82-57 DEFICIENCY EVALUATION 50.55(e)=

ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT 1, 2 & 3 I. DESCRIPTION OF DEFICIENCY-As discovered by the'Torrey Pines Technology QA' Evaluation of this project, drawings 13-P-ZZG-012 and 13-P-ZZG-013 require 3".through 8" globe. valves identified by mark number 350 to be constructed of SA351 GR CF 8M or SA182 GR F316 (stainless steel material).

Contirary to the above drawings, valves AFB-HB-30, AFB HV-31, AFA-HV-32 and AFC-HV-33 were supplied with bodies made of SA-105' and'SA-106 carbon steel material as per motor actuated valve data sheet 13J-081-060 Rev. 1. These valves'are installed at the auxiliary feedwater discharge lines to throttle demineralized water flow to the steam generators during an. Auxiliary Feedwater Actuation Signal (AFAS).occurence.

.The cause of this problem is attributed to an oversight whereby the Jvalve data sheet, prepared and~ issued by project engineering for construction, contained'the incorrect material requirements.

II. ANALYSIS OF SAFETY IMPLICATIONS Bechtel's Material and Quality Services Department (M&QS) agrees that the SA-105 or SA-106 valve body is not compatible with the piping class DCCA. M&QS estimated that the valve body will be satisfactory for three (3) to five (5) years. Corrosion will occur due to the domineralized water used in the system and by galvanic effects resulting from the dissimilar metal couple. If lef t uncorrected for some period of time the condition could possibly represent a safety significant condition, since a corroded valve body could fail resulting in the loss of auxiliary feedwater flow which would preclude the system from performing its safety related function. Therefore, this condition is evaluated as Reportable.

III. CORRECTIVE ACTION The carbon steel valve bodies will be replaced with stainless steel valve bodies. Replacement valves will be procured via a revised valve data sheet. Design Change Packages 10J-AF-025, 2SJ-AF-025 and 3CJ-AF-025 will be issued to implement corrective action.

The responsible individuals have reviewed this report and have performed a review of the other valve data sheets. This review indicates that this is an isolated case and will not reoccur since all the other safety related valve data sheets of this type have been coupleted and issued.
