Dear Mr. Keppler:
This is to confirm our verbal report to Mr. R. Dubry. INIC Region III Inspector. on December 30,1982,-et-appremiantely.4400 hours0.0509 days <br />1.222 hours <br />0.00728 weeks <br />0.00167 months <br />, at which time we informed Mr. Dubry of the location and nature of an obstruction in thit #2 valve 51-152-5 which prevented attainment of the minisus required flow through the south seflety injection header. The presence of a flow obstruction was indicated on December 23,1g82. at editch time performance testing showed a maximas attaineble flow of 260 gps (vs, twouired flow of 300 gpe, minieum) tW the flow path containing valve 51-152-5. Systesetic investigation since that date resulted in discovery of the obstrwetion (the unattached disc free a previous repair of valve 51-152-5) lodged within the valve body.
An investigation into the cause of this event has been initiated. A cosplete report will be submitted in fourteen (14) days.
Subsequent to the above December 30. Ig82 report, the obstruction was removed from valve 51-152-5 and the correct 51 flow rete was established
( with no further difficulties.
