Dear Mr. Keppler:
On October 20, 1982, Mr. T. D. Geib of Public Service Company of Indiana, Inc. (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e). During repair of spalled concrete around an embedded plate, deformed wire anchors were discovered to have separated from the plate. This item varies from that identified on August 18, 1982, since this embedded plate was installed after the resumption of safety related work at Marble Hill. Also, the scope of this problem is limited to separation of the wire anchors from the plate and at this time does not involve missing wires, nicked wires or wires bent beyond acceptable limits.
PSI is presently investigating the cause of plate / wire anchor sep-aration and will factor the results of this investigation into the development of a technique for examining the condition of deformed wire anchors on installed embedded plates.
This letter is intended to fulfill the requirements of an interim report as defined in 10 CFR 50.55(e). A further report will be filed on or before March 11, 1983. If you have any questions on this report, please feel free to contact me.
