The report recommendations concern:
a.
Resolution of Task Interface Agreement 81-15; b.
Reducing the probability of inadvertent opening of single valves 4
retaining the reactor pressure during RHR operation; and c.
Ensuring that procedures and training are available to operators for mitigating a LOCA during RHR operation.
