Thank you for your letter of July 13, 1982, in response to our letter dated June 18, 1982.
As a result of our review, we wish to inform you, as discussed with your Mr. P. L. Milinazzo during a telephone call on July 20, 1982, that we find your response unacceptable to us.
Improper design values with respect to stud torque requirements were furnished to your customer for a 12-inch, 1500 pound, motor-operated gate valve, which resulted, ultimately, in a discrepant product.
The values supplied were the result of the failure to make a necessary design revision during preparation of the design from a drawing for a 14-inch, 1533 pound gate valve, and brings into potential question the scope and adequacy of your design review process.
Accordingly, we cannot accept the statement in your response suggesting design changes were accomplished, when the evidence clearly indicates that required design changes were incomplete.
Please provide the corrective action measures which were originally requested by our letter of June 18, 1982, within 25 days from the date of this letter, in order that we may complete our review in a timely manner.
