O, Box 41 Lycoming, New York 13093 Gentlemen: Subject: NRC Inspection Report No. 50-333/91-06 A routine unannounced radiological controls inspection was conducted at the James A. FitzPatrick Nuclear Power Plant on March 11 - 15, 1991. Areas reviewed during the inspection included: training, Plant Occurrence Reports, high radiation area controls, contamination controls,and ALARA. Plant tours were also conducted. The findings were summarized and discussed with you and members of your staff during the exit meeting on March 11 -15, 1991. Within the scope of this review, one violation was identified. The violation involved a failure to lock an area where the radiation inten:;ity was greater than 1000 mrem /hr. It was determined that the violation did not meet the criteria for a non-cited violation as specified in 10 CFR 2, Appendix C. V. G., even though it was identified by a member of your staff, because a similar occurrence resulted in a non cited violation in December 1990. The violation is described in the enclosed Appendix A, Notice of Violation. You are required to respond to this Notice of Violation and follow the instructions specified in Appendix A. It did not appear that your use of the filters was consistent with the method described in your Final Safety Analysis Report (FSAR).
