Enclosed for your review, prior to our scheduled meeting of May 14, 1991, is the Initial SALP 12 Report for the Fermi 2 Nuclear Plant, covering the period January 1, 1990, through February 28, 1991.
In accordance with NRC policy, I have reviewed the SALP Board Assessment and concur with its ratings.
It is my view that your conduct of nuclear activities in connection with the Fermi 2 facility was acceptable and consistent with a conservative and safe operating philosophy.
This philosophy was demonstrated by your efforts to control shutdown risk and by your source term reduction initiatives.
In general, I believe your overall performance improved from the previous SALP period and I urge you to continue your efforts to improve your performance and ncte this is the second SALP period where performance overall has improved.
Nonetheless, areas remain, as noted below, where performance declined and your attention is needed.
1.
Plant operations, engineering / technical support, and safety assessment / quality verification were rated Category 2.
These areas were also rated Category 2 in the previous assessment period.
Maintenance / surveillance was rated Category 3 improving in the previous assessment period and improved to a Category 2 this period.
You are to be commended for applying the resources and efforts needed to improve your performance and resolve long standing deficiencies in that area.
2, Security, rated Category 1, improved from the previous assessment period in which it was rated Category 1 declining.
Your efforts to reverse the declining trend were acknowledged by the Board.
3.
Radiological controls was rated as Category 1 declining.
The Board concluded that overall your performance in this area was less effective.
While your ALARA initiatives were strong, problems persisted with contractor control and with untimely and inappropriate corrective action for unlocked high radiation doors.

Emergency preparedness declined from a Category 1 to a Category 2.
Although administration of the program remained essentially the same as in the previous assessment period, the demonstrations of your capability during the annual exercise and medical drill, which were conducted early in this assessment perio1, were weak.
Although efforts were successful in correcting the initial medical drill weaknesses, additional weaknesses were identified in subsequent medical drills.
Further attention will be needed to reverse the decline in this area.
At the SALP meeting, you should be prepared to discuss our assessments and your plans to improve performance. The meeting is intended to be a candid dialogue wherein any comments you may have regarding our report are discussed.
Additionally, you may nrovide written comments within 30 days after the meeting.
Your comments, a summary of our meeting, and my disposition of your comments will be issued as the Final SALP Report.
In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2 Title 10, Code of Federal Regulations, a copy of this letter and the Initial SALP Report will be placed in the NRC's Public Document Room.
