SUBJECT:
NOTICE OF VIOLATION (NRC INSPECTION REPORT N0. 50-313/90-42;
50-368/90-42)
This refers to the inspection conducted by Messrs. C. C. Warren, A. Singh, and
N. Terc, and Ms. L. J, Smith, of this office October 17 through December 7,
1990.
The inspection included a review of activities authorized for your
Arkansas Nuclaar One, Units 1 and 2, facility.
At the conclusion of the
inspection, the findings were discussed with those members of your staff
identified in the enclosed report.
Areas examined during the inspection are identified in the report.
Included
are onsite event followup, operational safety verification, surveillance,
maintenance, modification installation, outage activities, review of previous

inspection findings, and followup of licensee action on licensee event
i
reports (LERs).
Within these areas, the inspection consisted of selective
examination of procedures and representative records, interviews with personnel,
and observations of activities in progress.
Based on the results of this inspection, certain of your activities appeared to
be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice).
We are concerned about the violation because an inadequate
review of the safety evaluation for conducting resin transfer cask dewatering
activities in the train bay lef t the unit vulnerable to an unmonitored offsite
release during a recent resin fine spill.
Failure to perform an adequate
safety review is a violation of Technical Specification (TS) requirements.
A
similar violation was issued in the previous resident inspection report.
You
were fortunate that the lineup of air-handling units and the prevailing wind
patterns led to the containment of the resin fines.
This citation is being
issued because the inadequate review was not discovered prior to the
consequences occurrina.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence.
After reviewing your response to thic
l
Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
