Gentlemen:

This refers to the special inspection conducted by Messrs. A. L. Hon and

C. W. Caldwell of this office on October 1 through November 15, 1990.

This

inspection included a review of activities authorized for your San Onofre

Units 2 and 3.

At the conclusion of the inspection, the inspectors discussed

their findings with Mr. H. E, Morgan and other members of your staff identi-

fied in the enclosed report.

Areas examined during this inspection are described in the enclosed inspec-

tion report.

Within these areas, the inspection consisted of selective

examinatiors of procedures and representative records, interviews with

-

personnel, and observations by the inspectors.

Based on the results of this inspection, two apparent violations were

identified and are being considered for escalated enforcement action in

accordance with the " General Statement of Policy and Procedure for NRC

Enforcement Actions" (Enforcement Policy), 10 CFR Part 2 Appendix C (1990).

These two licensee-identified Technical Specifications violations involved

misalignment of valves that resulted in inoperability of important safety

equipment. One affected the emergency core cooling and containment cooling

sub-systems in Unit 3, and also affected the Unit's containment integrity.

The second violation affected the auxiliary feedwater system in Unit 2.

Since these violations are being considered for escalated enforcement action,

no Notice of Violation is presently being issued for these inspection find-

ings.

In addition, please be advised that the number and characterization of

the apparent violations described in the enclosed inspection report may change

as a result of further NRC review.

An enforcement conference to discuss these apparent violations has been

scheduled for December 11, 1990. The purposes of this conference are to

discuss the apparent violations, their causes and safety significance; to

provide you the opportunity to point out any errors in our inspection report;

to provide an opportunity for you to present your proposed corrective

actions; and to discuss any other information that will help us determine the

appropriate enforcement action in accordance with the Enforcement Policy.
