APPENDIX A NOTICE OF VIOLATION Mississippi Power and Light Company Docket No. 50-416 Grand Gulf License No. NPF-13 As a result of the inspection conducted on February 16 - March 11, 1983, and in accordance with the NRC Enforcement Policy, 47 FR 0987 (March 9, 1982), the following violations were identified.
A.
Technical Specification 6.8.1 requires written instructions be established, implemented and maintained.
Contrary to the above, the requirements of Administrative Procedure 01-S-06-3, Revision 10, paragraphs 6.1.2, 6.1.3 and 6.1.4 were not met in that on March 4,1983 three temporary alteration jumpers were not controlled by the jumper log and two lifted leads were not controlled as temporary altera-tions.
This is a Severity Level IV Violation (Supplement I).
B.
10 CFR 50, Appendix B, Criterion XVI as implemented by MP&L Operational Quality Assurance Program MPL-Topical-1, Section 16.5.3, and Plant Administrative Procedure 01-5-03-1, Revision 4, paragraph 6.2.6 requires prompt initiation and adequate documentation of quality related deficien-cies.
Contrary to the above, on February 21, 1983 the prompt initiation and adequate documentation requirement was not met in that the auxiliary hoist mechanical jam stop was manually bypassed and not documented as a quality related deficiency.
This is a Severity Level V Violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged viola-tions; (2) the reasons for the violations if admitted; (3) the corrective steps which have-been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.
