APPENDIX A NOTICE OF VIOLATION Duquesne Light Company Docket No.
50-334 Beaver Valley Unit 1 License No. DPR-66 As a result of the inspection conducted on July 12 - August 15,1983, and in (10CFR2.AppendixC) published accordance with the NRC Enforcement Policy (47 FR 9987), the following violation in the Federal Register on March 9,1982 was identified:
The BVPS Quality Assurance Program (Appendix A to the FSAR), Section A.2.2.14. Inspection, Tcst and Operating Status; ANSI N18.7-1972, Administrative Controls for Nuclear Power Plants; and QA Procedure OP-9, Technical Procedure Control for Operations and Maintenance, Revision 4, require the establishment of procedures for equipment control to maintain reactor and personnel safety and to avoid un-authorized operation of equipment.
OM Chapter 1.48.6, Section 6, Clearance Procedures, requires that en Equipment Clearance Permit be established prior to removing plant equipment or systems from service.
Contrary to the above, the "A" Reactor Plant River Water System was removed from service on August 5, 1983, without the required equip-ment clearance, when the header expansion joint was mistakenly identified and pulled as part of the "B" header in-service inspection.

This is a Severity Level IV Violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Duquesne Light Company is hereoy re-quired to subnit to this office within 30 days of the date of the letter transmit-ting this Notice, a written statement or explanation in reply, including for each violation:
(1) the corrective steps which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consi-deration will be given to extending the response time.
