' Docket No. 99900519/83-02 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted.on May 23-27, 1983, it '
appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendit B'to 10 CFR Part 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or...
drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with-these instructions, procedures, or' drawings.
Instructions, procedures, or drawings shall. include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Nonconfermance with this requirement is as follows:
SNUPPS Project Nuclear Quality Assurance Manual, Revision 2,: dated January 6, 1978, states, in part, " Requests for additions, deletions, or revisions to the Nuclear Quality Assurance Manual (NQAM) must be made in writing to the. Project.QA Manager with adequate justification for the proposed change.
When the Gaithersburg Power' Division Nuclear Quality Assurance Manual (GPD-NQAM) is revised, the revised portions will be reviewed to determine if a similar change should be made to the:SNUPPS Project NQAM....
Revisions will be subject to client review and approval."
Contrary to the above, the GPD-NQAM is at Revision 16, dated February 28, 1983, and has incorporated many changes which should be in the SNUPPS Project NQAM which is at Revision 2,' dated January 6,1978.
Revision'2 of the SNUPPS Project NQAM does not list the current Bechtel organizational titles.and the sections on supplier quality representative logs and trend analysis do not~
represent present practices.
Failure to keep-the~SNUPPS Project NQAM updated has been recognized by Bechtel i
and was reported-in QA Management Audit 10466-10.
A proposed revision to the SNUPPS Project NQAM, dated September 1982, has'been prepared.and. forwarded to th'e client for review.
No: approval-has been received toLdate.
At the present time an approved obsolete SNUPPS Project Nuclear Quality Assurance Manual is in existence ~which does not represent current QA practices.
