Appendix NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-237 As a result of the inspection conducted on March 16 through May 20, 1983, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9, 1982), the following violation was identified:
10 CFR 50, Appendix B, Criterion V states in part that " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, or a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Commonwealth Edison Company Topical Report CE-1-A, " Quality Assurance Program for Nuclear Generating Stations," Revision 14, dated September 9, 1980, states in Section 5 that " Activities affecting quality are required by the Edison quality program to be prescribed by documented instructions, procedures, or drawings."
Commonwealth Edison Company QA Manual Quality Procedure No. 5-51, " Instructions, Procedures, and Drawings for Operations - Station Procedures Manual", states in Attachment A, Revision I, dated July 10, 1975, section 5.6 that " Maintenance which can affect the performance of safety-related equipment shall be properly preplanned according to written procedures and in accordance with documented instructions, procedures, or drawings suitable to the circumstances.
Commonwealth Edison Company QA Manual Quality Procedure No. 3-52, " Design Control for Operations - Plant Maintenance," Revision 14, dated November 1, 1982, states in part that "the procedures to be used may be documented on Work Requests involving safety-related work.
Dresden Work Requests Nos. D26776, D26777, and D26778, which delineated maintenance work to be done on the inboard Main Steam Isolation Valves 1B, IC, and ID, respectively, included as part of the work instructions, directions to " Rebuild valve (s) per Vendor Manual".
The Crane Valve Vendor Manual referenced from the above Work Requests, in turn, stated that "Before the valve is reassembled, make sure all parts are clean".
Dresden Work Requests Nos. D25246 and D25819, for work on the 2A and 2B inboard feed water check valves states under work instructions that "A reactor feed pump discharge check valve manual is attached to be used as an aid."
That manual gave specific instructions to inspect and clean the lower disc assembly using denatured alcohol, rags, fine emery, wire brush and stones.

Contrary to the above, the Vendor Manuals and repair manual procedure were not followed by maintenance personnel in that grease was applied to the main disc / seat' mating surfaces prior to reassembly of the IB, IC, and ID, Main Steam Isolation Valves and the-2A and 2B reactor feed water inboard check valves.
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) correc-tive action taken end the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Consideration may be given to extending your response time for l
