- 1. Equipment. and Operator Response (J. L. Seelinger) l The events following the reactor tr',p wre sumartred in chronological order by the age of curve made up fras transient data. Conclusions and reconnendations were then presented in the areas of 1) operator.
actions; 2) equipment response; 3) philosophy; 4) training; 5) and others. In retrospect, it was noted that the reactor trip on ths Wednesday preceding this trip indicated that the main staas relief valves were not reseated properly, however, the main emphasis ws en the steam generater level and the data was incomplete.
From the recomendations. two lists were made up; 1) contafof items.,

There was further discussion on the oferophone moattor of the relief valve and R. J. Toole will examine possible sound monitor to light-up alarm board vs. audible indication. Also walk through program at shift briefing of any major evolution and manual actions required to, correctly respond was examined.

Main Steam Relief Valve (R* J. Toole). The vertical'11ners will be examined with tM use of a T.V. camera system and one Ifner will be dye penetrant tested. If vertical appear sound then they will not be removed. Ef'i will have to sutrsit design acceptability and GPU will need to present field data fres examinations. During the HOT shutdown condition (% 540'f) the M5PV will be If f ted and tested for their ressating pressure. Afterwards, they will be monitored during the 405 power reactor trip for acceptanle operation. Pathway feels the bellows are not good enough to handle steam flow and BIR is checking the design conditions out. If any damage is noted on any vertical liner it will be replaced. John Erwwwr added that videotapes of the vertical ifner inspection should be made for future reference.
