Dear Mr. Keppler:
This letter serves as a second interim report pursuant to 10CFR50.55(e) concerning violations of specification requirements during installation of adjacent Hilti Kwik bolts.
Initial notification of this problem was made to Mr. F. Jablonski of your office on February 7,1983, by Mr. J. Kerr for The Cleveland Electric Illuminating Company. Our first interim report on this subj ect, filed March 9,1983, indicated that our next report would be filed July 1, 1983.
This report contains a description of the deficiency, corrective action taken to date, describes the method planned to complete our evaluation, and the planned date for tLa final report.
Description of Deficiency During field inspections of some installed Hilti Kwik bolts, it was determined that the minimum spacing requirements established by the installation attachment specifications, SP108/208, have not been met in ali cases. This condition is documented on Nonconformance Report CQC 2644 and involves the spacing between adjacent Hilti bolts, grouted-in bolts and the free edge of concrete members.
Corrective Action Taken To Date All contractors installing Hilti bolts were informed by the Project Organization by letters dated January 28, 1983, and March 10, 1983, of the proper interpre-tation of spacing requirements from installation specifications SP208/108, respectively, which is to be applied for all future work. The affected contractors have been informed that any apparent conflict between drawing requirements and the spacing requirements of SP108/208 shall be brought to the attention of Project Organization Engineering for resolution prior to ins talla tion.

July 1, 1983 Method Planned To Complete Our Evaluation On-site load tests of. multiple, closely spaced Hilti Kwik bolts have been -
performed. Two (2) test' areas were selected in the plant to represent all' 4 -
PNPP safety-related Category I concrete.
As a result of - the testing, program, we are in t o process of revising speci-b fications SP108/208 to incorporate revised spacing requirements for 1/4",
3/8",;5/8", '3/4" and 1-1/4" diameter Hilti Kwik bolts by issuance of ingineering Change Notices. The specifications were previously revised to prohibit the use of 1/2" diameter Hilti Kwik bolts. The Engineering Change Notices now in process will prohibit the use of 1" diameter Hilti Kwik bolts.. A case-by-case

evaluation cf all installed 1/2" diameter and 1" diameter Hilti Kwik bolts will be performed.
An inspection program to verify the spacing between adjacent Hilti-Kwik bolts, I
grouted-in bolts, and the free edge of concrete members will be developed and
-implemented by Project Organization, Construction Quality Section. _ This inspection program will include all Hilti Kwik bolts installed prior to the issuance-of the Engineering Change Notices.
The planned date for the final report on this subject is now September 30, 1983.
