W STIPULATION REGARDING RECENT FINE IMPOSED UPON COWOWEALTH EDISON On April 11, 1983, Edison received a notice of violation with respect to an occurrence at its Quad-Cities Station.
Edison failed to maintain the operability of one of two 16strument channels in the "B" trip systen which generate the turbine pressure generator load rejection and stop valve sc' ram bypass, through forty hours, five operating shifts, even though an annunciator was continuously lit on the control panel in the control room indicating a malfunction in that system.
The transmittal letter dated April 11, 1983 and signed by James G. Keppler, Regional Ad'ministrator, states:
"lle are concerned that the degraded condition of the reactor protection system which was annunciated in the control room went unnoticed i
for an extended period of time."
Operating in this mode was contrary to the technical specifi-cations to the operating license for the Quad-Cities Station.
A civil penalty in the amount of $60,000 was proposed for this violation.
The base value for this Severity Level III violation is $40,000, which was increased to $60,000 because of the 830705oo32 930629 DRADOCK05000g 4

duration of the violation and because the operators should clearly have been aware of the condition inasmuch as the control room annunciator was lit.
In addition, during the same inspection, a Severity Level IV item of noncompliance was identified in connection j
with failure to control overpressurization devices during a hydrostatic pressure test.
