due to the event's showing that the unit was not meeting the requirements of Tech.

Specs. section 4.5.1.c.1.

Plant conditions at the time of the event (s):

On April 6, 1983, the plant was in cold shutdown for a refueling outage.

Detailed description of the-event (s):

B During performance of "HPCI AUTOMATIC ACTUATION" procedure (HNP-2-3322),

the HPCI pump discharge valve (2E41-FOO6) failed to open.

Consequences of the event (s):

The plant was in cold shutdown at the time of the event.

HPCI is not required to be operable in cold shutdown.

There were no consequences of this event.

The health and safety of the public were not affected by this event.

Status of redundant or backup subsystems and/or systems:

'No redundant or backup systems are required in cold shutdown.

Justification for continued operation:

Unit was in cold shutdown during refueling outage and HPCI was not required to be operable.

If repetitive, number of previous LER:

This is a non-repetitive event.

Impact to other systems and/or Unit:

This' problem does not affect the other plant systems, nor does it affect Unit 1.

Cause(s) of the event (s):

The cause of the event was attributed to a loose wire located on the motor terminal block inside of the value operator.

The cause of the loose id re that prevented the 2E41-FOO6 value from operating is unknown.

The loose wire was tightened and 2E41-F006 valve was cycled.
