APPENDIX A Garrett Pneumatic Systems Division Docket No.. 99900370/83-01' NOTICE OF NONCONFORMANCE
. Based on-the results of an NRC inspection conducted on' January 31-FeDruary 4, 1983, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10:CFR Part 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate -to <the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Nonconformances with these requirements are as follows:
A.
Paragraph 3 of Garrett Operational Procedure No. 4.4, " Nuclear Hardware-Posting and Reporting Requirements," in the Product Integrity Manual dated June 17, 1981, states, "The requirement for reporting defects or noncompliance to the Commission becomes effective after the first part has been shipped."
Contrary to the above, the Nuclear Regulatory Commission was not formally notified pursuant to 10 CFR Part 21 in regard to the determination that shipped nuclear solenoid valver contained a defect.
B.
Subparagraph 5.2.5 in paragraph 5.0 of Combustion Engineering Vendor Quality Control Program Specification No. 0000-WQC 11.1 states,
" Identification and Control of Materials, Parts and Components - System for identification and control designed to prevent the use of incorrect or defective materials, parts or components.
Identification shall be by' heat number, part number, serial number on the equipment, or by equipment records i
or other appropriate means - traceable to the item."
Contrary to the above, the QA program did not include an identification and control system for providing traceability of nonpressure boundary safety-related items.
