Gentlemen:

This refers to the inspection conducted by Messrs. C. C. Warren and R. C. Haag

of this office during the period February 1 through March 15, 1990, of

activities authorized by NRC Operating Licenses DPR-51 and NPF-6 for Arkansas

Nuclear One, Units 1 and 2, and to the discussion of our findings with you and

other members of your staff at the conclusion of the inspection.

Areas examined during the inspection included followup on pmviously identified

items, followup of events, operational safety verification, surveillance,

maintenance, and in-office review of licensee reports.

Within these areas, the

inspection consisted of selective examination of procedures and representative

records, interviews with personnel, and observations by the inspectors.

The

findings are documented in the enclosed inspection report.

l

During this inspection, it was found that one of your activities was in

j

violation of NRC requirements. Although we recoginize that the violation

.l

was identified and reported by your staff, it is a repeat of a continued

i

I

pattern of the failure to perfom surveillance tests within the required time

i

L

interval for which your corrective actions, to date, have not precluded

,

I

recurrence. Consequently, you are being cited in accordance with

j

paragraph V.G.I.e. Appendix C, Part 2. Title 10. Code of federal Regulation,

'

and you are required to respond to this violation, in writing, in accordance .

,

l

with the provisions of Section 2.201 of the NRC's " Rules of Practice," Part 2,

i

Title 10 Code of Federal Regulations. Your response should be based on the

,

spet.ifics contained in the Notice of Violation (Notice) enclosed with this

l

letter.

l

In addition, during the upcoming quarterly performance meeting scheduled for

t

April 26, 1990, you are requested to discuss the actions you have taken to

!

improve your surveillance test program controls. Also, because of the pattern

j

of recent problems with equipment controls, such as the failure to tag-out a

j

diesel generator prior to commencing maintenance, you are requested to discuss,

l

during the meeting, your equipment control program and the improvements in

!

controls that you have made or plan to make.
