Subject: Combined Inspection Report Nos. 50-54/90-80 and 70-687/90-80
This letter refers to the special team inspection conducted on February 9-16 and 26,
1990, by the NRC in response to the discovery of leaks from the reactor pool system
on February 9,1990. The inspection also reviewed your efforts in response to the
contaminated storm drain system, that apparently resulted from leaks in the
' underground ventilation duct from the hot cells. As part of this inspection, the NRC
Mobile Radiological Measurements Laboratory was brought to your site and used for
independently assessing radiological conditions and verifying your radioanalytical
. capability. The inspection consisted of document re'.icsys, interviews and observations
of activities, and measurements made by the team. The findings were summarized and
discussed with you, Mr. Gallaher and members of'your staff on February 15,1990 by
Drs. Robert J. Bores and Ronald R. Bellamy of my staff.
One violation of NRC requirements was-identified following the exit on February 15,
1990. You were informed of this violation in. discussion on February 26 between you
and Dr. Bores. You are required to respond to the violation in accordance with the
enclosed Appendix A. That reply is not subject to clearance by the Office of
. Management and Budget under the Paperwork Reduction Act of 1980, PL 96-511. We
note that you have taken short-term corrective actions to prevent or minimize
recurrence and a Confirmatory Action Letter (CAL 1-90-005) was issued to confirm

certain of those actions. Some of those actions were reviewed by the NRC on
February 26,1990. Consequently, in your response you need address only those
additional short-term actions and your longer term actions and plans you have taken or
will take to ensure there will be no recurrence.

The storm drain contamination appears to have been the result of leakage of hot cell
exhaust past the main filter bank. As a result, an unresolved item was identified
relative to the collection efficiency of the filter banks with a bypass leakage path. Since
this item is related to an unresolved item identified in a previous report for which you
have been requested to respond, please incorporate your response to this new concern
with your response to that item (70-687/89-80-13).
