NOTICE OF VIOLA. TION GPU Nuclear Corporation Docket No. 50-289 Three Mile Island, Unit 1 License No. DPR-50 During an NRC inspection conducted between December 19, 1989 January 23, 1990, i

a violation of the Technical Specifications (TS) was identified.

In accordance with the " General Statement of Policy and. Procedure for NRC Enforcement p

. Actions", 10 CFR Part 2, Appendix C, the violation is set forth below, t

Technical Specification 6.8.1 requires that written procedures shall be i

established and implemer,'.ed for surveillance and test activities of equipment that affects nuclear safety.

Contrary to the above, on January 6,1990, the licensee failed to adequately establish and implement Surveillance Procedure (SP) 1303-11.18B " Reactor Building (RB) local Leak Rate Testing - RB Spray Pressure Instrumentation".

l Step 6.5 did not provide adequate and complete guidance for resetting the "A" Reactor Protection System (RPS) channel. This resulted in an inadvertent RPS actuation. Additionally, step 6.11 which resets Engineered Safeguards Actua-tion System (ESAS) Channel 3B was improperly completed in that this resulted

-in an inadvertent Engineered Safeguard Actuation System actuation.

Pursuant to the provision of 10 CFR 2.201, GPU Nuclear Corporation is hereby required to submit to this office, within-thirty days of the date of the letter transmitting this Notice, a written statement or explanation in reply, includ-t ing: (1) the corrective steps which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further violations; and, l

(3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
