  • The Inservice Inspection was performed during the Unit I third refueling outage: which began September 15, 1989. - The scope of the inspections ' included ASME Section XI required examinations and N.R.C.

required augmented ~ examinations. All examinations performed were in accordance with LaSalle Unit 1 Technical Specifications and ASME Section XI 1980 Edition including the Winter 1980 Addenda. General Electric Co. was contracted to psrform the Non-Destructive

Evaluations-(NDE) of the reactor vessel welds, piping welds and pressure boundary att achment welds. Magnetic particle (MT), liquid dye penetrant (PT)-and manual or automatic ultrasonic (UT) techniques were used during
                      -these inspections. Personnel from Commonwealth-Edison's System Material
                      ' Analysis Department (SMAD) and LaSalle Station's Technical Staff provided any necessary resolution of technical problems, reviewed all I.S.I.

personnel qualification, and reviewed all surface and volumetric examination results. 1The Level II and III inspection personnel performing UT were qualified-for detection and discrimination of I.G.S.C.C. by the Electric Power Reteach Institate (EPRI) at the EPRI NDE center after September 10, 1985.- Level I inspection personnel not qualified at EPRI did not perform UT. scanning duties, although they were used as helpers in the set-up of the automatic UT. They were trained in set-up' activities by EPRI' qualified G.E. personnel on site. Visual examinations of reactor vessel internals, class 1 bolting and piping pressure tests', were performed by. CECO, G.E. and Sargent and Lundy employees. . These. Inspectors were currently certified as ASME Section XI Visual Examiners through the CECO qualification program. An underwater camera was used in the reactor vessel for the visual inspection of the reactor vessel internals. The camera was proven capable of resolving a 1 mil stainless steel wire underwater in the same lighting conditions found during the actual inspections.

                               'The services of an Authorized Nuclear Inservice Inspector

( ANII)/ Authorized Nuclear Inspector ( ANI) were provided by Hartford Steam Boller Inspection & Insurance Co., 300 S. Riverside Pl., Suite 700 N., Chicago, IL. 60606. The ANII/ANI, John Tetreault, National Board 18796, State of IL. 81416,' reviewed all procedures, personnel qualifications, instrument and material certifications, all examination results, and repairs and replacements. II. SCOPE OF TNSPECTIGtS The tables following this section summarize the extent and results of the-inspections petformed during the LaSalle Unit 1 Fall 1989 refueling outage. An explanation of any abbreviations can be found at the end of this report. Table A provides a summary listing of all completed ASME Section XI examinations for this outage, except for pressure testing and component support-inspections, which are both addressed separately. Table A is divided by Section XI categories, and where applicable, gives the percentage of category examinations taken credit for thus far in the inspection interval. A description of the item examined is given as well as the examination type (s) and examination result (s). 1 ZCADTRNG/40

        .,     o Table B provides a summary listing of all completed ASME Section XI component support exnminations for thia outage. Table B is divided by Section XI categories and given the percentage of category examinations taken credit for thus in the inspection interval.                     A description of the item examined is given as well as the examination type and examination result (s).

Table C is a listing of all I.G.S.C.C. susceptible welds inspected this outage as a result of H.R.C. Generic Letter 80-01. A minimwn number of 22 susceptible welds were required to be examined. The minimum was exceeded as 23 welds considered to be susceptible by N.R.C. Generic Letter ' B0-01 guidelines were inspected this outage using ultrasonic examination techniques. Table B lists those welds exwnined as well as the results of the UT examinations. The ASME Class 1 piping and components, within the boundary as defined per IWA-5221 of ASMC Section XI, was inspected during the system leakage test at the reactor nominal operating pressure of 1020 PSIG. The ASMC Dection XI coiaponent pressure testing of ASME Class 2 and 3 vill continue throughout the second inspection period. Since the commercial operating date of 1-1-84, LaSalle Unit I has completed repair / replacements on Unit 1 in accordance with IWA-4000 and IWA-7000. This work was completed in accordance with LaSalle Administrative Procedures LAP-1300-1 and LAP-1300-13, and are on Ille at LaSalle. The name of each major component inspected will be listed on the form NIS .' f or Colwnn 11. Only a list of the major components inspected will be listed on the form NIS-1. These components include the Reactor Vessel, the RHR Heat Exchangers, the Class 1 & 2 piping Systems and the Class 1 & 2 Compenent Supports inspected during the refueling outoge and will be listed on the form NIS-1, Item 50. A more detailed list of components inspected will appear in the abstracts listed as items 10 11 and 12 on the form NIS-1. Since descriptions including site, capacity, matcrial, location and drawings to aid in identifiention have been previously submitted in our N-5 data reports and also our I.S.I. program, they will not be included in the summary report but are on Ille at LaSalle. This also applies to our preservice and previous inservice inspection summary reports. l 2CADTRNG/40

         ?                                The ASME Section XI inservice inspection results obtained this outage were compared with the preservice and previous inservice inspection results. . The following is a summary of the results and the~correctiro actions taken this outage.
1. THE S*ln.4 DPNFR Dut bag rbe Unit 1 third refuel outage, inservice inspection of the steht atyer was performed. Tht. results showed 12 crack-like linear indications in 9 different locations on the steam dryer. CECO's Nuclear Engineering Department and General Electric's Engineering Services reviewed the visual inspection
                                          -data for the crack indications found in the steam dryer and concluded they are acceptable for one more cycle of operation and do not need repair before start up.               At the beginning of the next (fourth) refueling outage LaSalle will perform a visual
                                          . inspection of the dryer to determine crack propagation and potential need for repair.
2. COlEPGIENT SUPPORTS Constant Support RH04-1505C was observed to have its travel stops installed and locked during the performance of an I.S.I.

VT-3/4 visual examination. A CECO Nuclear Engineering Department evaluation concluded that no piping overstress occurred.and the resulting fatigue damage was within acceptable limits and that support. RH04-1505C may be placed into service. Due to the condition observed above, the I.S.I. Component Support Inspection sample was expanded per ASME Sectivn XI. Because of the large initial sample of constant supports all five of the remaining constant supports in the'RHR system were einspected. Of those 11ve supports the following supports were found acceptables RH04-1002C

                                                                        -RH04-1006C RH04-1004C RH53-1002C During this sample expansion another support was found with its travel stops in place and locked. Component support RH53-1001C was unlocked and stroked through its thermal range and no af fect to operability was found. CECO's Huclear Engineering Department evaluated this condition and concluded that no piping overstress occurred and the resulting fatigue damage was within acceptable limits.             Support RH53-1001C was put back in service.

As a result of this support being found pinned, additional systems were walked down in the drywell. RHR "A", "B", "C", SC, RT, HP, LP, MS, and RR had all their constant supports inspected for installed travel stops. 3.- LASALLE'S "I.G.S.C.C." STATUS During this refueling outage, the G.E. automated " SMART" UT-system redefined the previously known I.G.S.C.C. -'like indications in welds IRR-1001-10 and ORR-1005-27A ac Root and I.D. Geometry. This redefinition of_the previously known I.G.S.C.C. - like indications is attributed to the additional machining of the weld crown on these-two (2) welds during this refueling outage. The machining of the weld crowns resulted in a smoother scanning surface for the UT system.
