APPENDIX A NOTICE OF VIOLATION Portland General Electric Company Docket No. 50-344 121 SW Salmon Street, TB-17 License No. NPF-1 Trojan Nuclear Plant During an NRC inspection conducted from December 4,1989 through January 31, 1990, a violation of NRC requirements was identified.
In accordance with the
" General Statement of Policy and Procedure for NRC Enforcement Action", 10 CFR Part 2, Appendix C (1989), the violation is listed below:
10 CFR 50 Appendix B Criterion V states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings."
Trojan Maintenance Procedure MP 2-0, " Installed Plant Instrument Maintenance and Calibration", Revision 12, effective date September 21, 1989. Section Note 6.2.6.d. states in part, "If the as found readings for quality-related instruments are outside the stated tolerance...The I&C l
technician shall... bring it to the attention of the 180 supervisor or his designee. They shall initiate a Form 180-10, Installed Instrument Out-0f-Calibration Investigation (see Section 6.3) for Quality-related instruments."
l Cor,trary to the above, on May 5, 1989, when the Emergency Diesel l
Generator lube oil filter inlet temperature instrument (No. TI-3821X) failed its scheduled calibration test, a Form I&C-10 was not initiated.
l This is a repeat Severity Level IV Violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Portland General Electric Company is hereby required to submit a written statement of explanation to the U.S.
Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555 with a copy to the Regional Administrater, Region V, and a copy to the NRC Senior Resident Inspector, Trojan, within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a
" Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation if admitted. (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken. Consideration may be given to extending the I
response time for good cause shown.
