This refers to the routine resident safety inspection conducted by Messrs.
W. Raymond and K'. Kolaczyk of'this office on November 28,'1989 through.
. January 4,-1990 at Millstone. Nuclear-Power Station, Unit 3. Waterford,
. Connecticut of activities authorized by NRC License No. NPF-49 and to the
- discussions of our findings held by Mr. Kolaczyk with Mr.'C. Clement and
others of your staff at the conclusion of the inspection.
Areas examined during this inspection are described .in the NRC Region I
,
inspection report which is enclosed with this letter. Within these areas, the

- inspection consisted of observation of activities, interviews with personnel,
I
measurements and document reviews.

Based ~ on. the ^results of this-inspection, it appears that one of your
')
' activities:was not conducted in full compliance with NRC requirements, as
tet..forth in the Notice of Violation enclosed herewith as. Appendix A. The
violation concerned the' removal of the. automatic start capability of the
service water booster pumps, (1) without determining whether the change
j
involved an unreviewed safety question,.and,(2) without changing the service
"
water operatir.g procedure 3326A. This violation has been categorized by
i
severity level in accordance with the " General Statement ofzPolicy and
!
Procedure for NRC' Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement
'!
Policy).
You-are required to respond to this letter and in preparing your
j
response,'you should follow the instructions in Appendix A.
'
In addition, another one of your activities apparently was not conducted in
.i
full compliance with NRC and Department of Transportation requirements.

~ The' apparent violation involves a Millstone Station shipment of a limited
i
quantity of radioactive materials that, upon receipt at the Haddam Neck Plant,
.
-had a dose rate on contact with the package in excess of the 49 CFR limits of
1
0.5 mr/hr. This matter is under NRC staff review and will be addressed in
separate correspondence.

With regards to matters discussed in sections 6.1 and 6.2 of the enclosed in-
spection report, we are concerned about the apparent untimely notification and
reporting of the potential problems with the 4160 volt fast bus transfer
scheme. That problem was determined to be reportable on November 27, 1989 as
a result of a reportability evaluation form initiated on October 20, 1989. We
note the comprehensive analysis and assessment of the problem by your corporate
,
engineers. However, it appears that the discovery of the condition occurred by
July 6,1989, when engineering reviews were completed sufficient to conclude
that there was a real, albeit low, probability of an event.
Our inspector discussed these concerns with your station management and was
told that a similar issue was noted by Northeast Utilities management and was
being addressed.
You are requested to respond to this concern.
Please submit
to thi', office, within thirty days of the date of this letter, a written state-
ment or explanation in reply, including; (1) your assessment of when the fast
transfer problem should have been reported and (2) the steps which have or will
be taken to assure the timely notification and reporting of events or condi-
tions as required by 10 CFR 50.72 and 10 CFR 50.73.
The response directed by this letter and the accompanying Notice is not subject
to the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Act of 1980, PL-96-511.
