Environmental. Radiological and Meterological Monitoring is submitted as Part III of the Zion Station Annual Operating Rerart.

This report will be transmitted under separate cover.

II. Waukegan Memorial Airport Expansion Plans:

i The airport master record (FAA 5010-1) and airport map (FAA 5090-1) has been updated and submitted by the Haukegan Port Administrator.

The only change being a change in the airport Manager.

This position is currently being filled by Mr. A. L. Huisman.

Ongoing expansion plans at the Haukegan Regional Airport are; land acquisition for runway 05 approach and obtaining aviation easements in j

the same area. i.e. Greenbay Road (East and West side).

III. Annual Occupational Exposure Data is reported on Personal Radiation Exposure per Reg. Guide 1.16.

This report will be transmitted under 1

separate cover.

IV. Challenges to Primary System PORVs and Safety Valves:

No challenges occurred to either the Primary System PORVs or Safety Valves during this reporting period.

This report fulfills the requirements of NUREG 0737 item II, k 3.3, " Reporting S/RV Failures and Challenges".

Unit i refueling outage started on 9/7/89 and ended on 1/25/90.

1 Fuel was loaded in 1989.

Modifications:

i Five reportable modifications were completed and verified to operate properly. The modifications are:

Modification Description - M22-1-88-029:

Portions of CRDM penetrations K-4 and G-5 have been cutoff and capped so that primary leakage can be prevented.

Leakage was identified during the Unit I refueling outage.

Safety Evaluation Summary:

L Modification ensures integrity of the Reactor Coolant System

pressure boundary. Only parameter changed is the location of the pressure boundary.

No new accidents created and the modification is bounded by previous accident analysis.

Modification does not affect margin of safety as the stress l

Modification Description - H22-2-84-028:

Replaces Powell Gate Valve RV0005&6 with post-seal high performance butterfly valves in the Containment vacuum and pressure relief line.

Safety Evaluation Summary:

These valves provide Containment isolation function for the

vacuum and pressure relief line.

Dual valve isolation provides redundancy to maintain Containment leakage below Technical Specification limit of 11. per day.

The valves are designed fall i

l safe-closed for all accidents, loss of air and electrical l

power.

New valve testing requirements will be maintained as l

required by Technical Specification 3.9.

Modification Description - M22-2-86-023:

Rearranges the Bypass and Permissive Panel in accordance with the human factors design review.

Safety Evaluation Summary:

The modification only involves an annunciator alarm and does not affect any accidents previously analyzed.

It also does not affect a margin of safety as described in the Technical Specifications.

Modification Description - M22-87-055:

j Install flanged spool pieces into area of piping which j

previously contained air filter canisters.

The spool pieces are i

designed to maintain the seismic qualification of the associated piping system.

Piping system is non-safety related.

Safety Evaluation Summary:

System is described in FSAR section 11.3.3, but is not used because it does not meet the requirements of NUREG 0737 regarding post-accident sampling.

The portion of.the system addressed is non-safety related and does not perform any post-accident functions.

However, it is required to be seismic qualified.

Modification Description - M22-1-82-35:

i The MSIV hydraulic system is being changed to satisfy i

L Environmental Qualification (EQ) and improve logic interlocks.

Hydraulic fluid will be used to open the valve, but N2 gas pressure will be used to close the valve.

The partial stroke remains the same and closing time requirements remain fulfilled.

Safety Evaluation Summary:

i This change improved the system's reliability because of the Equipment. Qualification program.
