APPENDIX A NOTICE OF VIOLATION Wolf Creek Nuclear Operating Corporation Docket:

50-482 Wolf Creek Generating Station License: NPF-42 L

During an NRC inspection conducted January 1-31, 1990, two violations of NRC requirements were identified.

The violations involved a failure to perform a safety evaluation and inadequate corrective action regarding conditions adverse to quality.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1989)

(Enforcement Policy), the violations are listed below.

A.

Failure To Perform a safety Evaluation Title 10 CFR Part 50.59, paragraph (b)(1), requires, in part, that records of changes must include a written safety evaluation which provides the bases for the determination that the change does not involve an unreviewed safety question.

Contrary to the above, on January 2,1990, the inspectors identified that the licensee temporarily removed a missile shield to one of two trains of essential service water, which constituted a change to the facility as described in the safety analysis report, without first performing a written safety evaluation. This is a repeat of a previous violation (482/890F01).

This is a Severity Level IV violation.

Inadequate Corrective Action on Chainfalls Title 10 CrR Part 50, Appendix B, Criterion XVI, " Corrective Action" st6tas, in part, that in the case of significant conditions adverse to qui,11ty, the measures shall assure that the cause of the condition is determined anc co<rtetive actice taler to pret tude vnpetitien.

Contrary to the above, on January 12, 1990, the inspectort identified a repeet condition adverse to quality for which inadequate correcthe action had been taken in that the inspectors observed a chai'nfall tied off to the fuel oil supply line to "B" diesel generator, a condition potentially affecting the operability of the diesel generator, which was the repeat of a condition identified to the licensee on March 7, 1988.

This is a Severity Level IV violation.
