1 NOTICE OF VIOLATION i

During an NRC inspection conducted October 12 through November 30, 1989, a violation of NRC requirements was identified.

In accordance with the " General Statement of Polic and Procedure for NRC Enforcement Actions " 10 CFR Part 2, j

Appendix C (1989) y(Enforcement Policy), the violation is listed below:

j Failure to Comply with the TS LCO for the-Inoperability of Two CCW Heat Exchancers TS 2.3(2) states, in part, that during power o)erations, the minimum requirements may be modified to allow one of t1e following conditions to exist.

If the system is not restored to meet the minimum requirements within the period specified, the reactor shall be placed in a hot. shutdown condition within 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />. If the minimum requirements are not met within t

an-additional 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br />, the reactor shall be placed in a cold shutdown

. condition within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

TS 2.3(2)c, a subsection of TS 2.3(2), states, in part, that two of four CCW heat:exchangers (HX) may be inoperable for a-period of no more'than 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />, TS 2.3(2)d, a subsection of TS 2.3(2), states, in part, that any valves directly associated with a component (i.e., a CCW HX) and required to function during accident conditions shall be deemed to be part of that component.

Contrary)to the above, the licensee failed to comply with the LC0 for TS.2.3(2 in that CCW HXs-AC-1A and AC-1B were. inoperable for.6 days and the plant was not placed in a cold shutdown condition within the 108 hours0.00125 days <br />0.03 hours <br />1.785714e-4 weeks <br />4.1094e-5 months <br /> (4.5 days) required by TS 2.3(2).

This is a Severity Level IV violation.
