APPENDIX A NOTICE OF NONCONFORMANCE During inspections conducted on February.7-10, 1989, the implementation-of the Quality Assurance.(QA) program at the Hardware Specialty Company, Incorporated,

-(HSC) in Long Island City, New York, was reviewed with respect to the supply of material to the nuclear industry. The applicable QA program requirements are Appendix B to 10 CFR 50, 10 CFR Part 21,Section III of the ASME Boiler and Pressure Vessel Code, and various HSC manuals and procedures.

Based on the results of the inspection, it appears that activities at HSC were not conducted in accordance with these commitments. These items are listed below, l

Pa'ragraph NC-2311 of ASME Section III,1977 Edition, Summer 1979 Addenda requires that Charpy impact tests be performed on fasteners greater than 1 inch.

y Contrary to the above, HSC-certified material test reports (CMTRs), dated j

April-27, 1982, did not provide impact test results on ASME SA-193, Grade B7 1 1/16-inch diameter fasteners supplied to LPL purchase order (PO) LO1478, dated December 30,1981.-(89-01-01) 2.

Paragraph 10.2.3 of SA-193 of ASME Section II and Subsection NC of ASME o

j'Section III,1977 Edition through Sunner 1979 Addenda requires that Grade B8, Class 1 bolts not exceed a maximum hardness of 223 Brinell.

Contrary to the above, HSC issued a Certificate of Conformance (C of C) to LPL indicating the' bolts complied with the requirements for SA-193, Grade B8 bolts. The bolts were supplied to HSC by the Fry Steel Company. Fry Steel reported the Brinell hardness for this material as 241 to 248, which exceeds the maximum allowed hardness. (89-01-02) 3.

Subsection NCA-3867.4(a) requires that CMTRs report specific times and temperatures of heat treatment of materials when required by the material specification. The material specifications for SA-193, Grade B8 and SA-194, Grade 8 material require that the material receive a carbide solution treatment and that the manufacturer certify that all requirements hcVe been complied with.

Contrary to the above, HSC issued C of Cs for SA-193 studs and SA-194 nuts supplied to LPL which stated that the material had been carbide solution treated with no documented basis in the HSC files for the C of C heat treatment statement.

Subbettion NCA-3867.4(e) of ASME Section til requires that stock Material that.is required to be supplied with a CliTR (bolting greater than 1 inch in diaracter) be upgraded by testing each piece of material or by establishing trace 6bility by surveying, qualifying, and auditing the Quality System Programs of HSC vendors.

Contrary to the above HSC sold bolting greater then 1 inch in diameter 1

certified as meeting ASME Section 111 requirements to LPL for use at Waterford with no indication on vendors' CMTRs tact the material was produced under the requirements of ASME Section 111 or that the upgrade t

requirenents of NCA-3867.4(e) had been met by HSC. (89-01-04)
