Dear Mr. Lewis:
This letter addresses the inspection of your facility at Long Island City, New York, conducted by Messrs. R. L. Cilimberg, I. Barnes, and A. Pjura of this office on February 7-10,.1989, and the discussions of their finding: with you and other members of your staff at the conclusion of. the ir.spection. Release of this report was delayed during NRC's ongoing review nf nonconforming-and substandard vendor products.
The purpose of the inspection has to follow up an NRC inspection at Waterford 3 which determined that Hardware Specialty Company, Incorporated (HSC), issued documentation to Louisicna Power and Light (LPL) on gland studs that were supplied to LPL'in 1981 which did not meet LPL requirements. Areas examined during the NRC inspection and our findihgs are discussed in the enclosed report. This inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspectors.
-During this inspection it was found that the implementation of your QA program failed to meet certain NRC requirements. HSC-supplied material to LPL in 1981 with incidents of Charpy impact tests not performed, specified hardness not met, heat treatment temperature not stated, each-piece of material not tested and HSC vendors not audited. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.
Please provide us within 30 days from the date of this letter a written state-ment containing:
(1) a description of steps that have been or will be taken
- taken to prevent recurrence; and (3)ption of steps that have been or will be to correct these items; (2)'a descri the dates your corrective actions and
-preventive measures were or wi,11 be completed. We will consider extending the response time if you can show good cause for us to do so.
The response requested by this letter is not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
