SUMMARY
This is the final report of Metropolitan Edison Company in response to USNRC IE Bulletin No. 79-14, dated July 2, 1979, including Revision 1 dated July 18, 1979, ,
Supplement I dated August 15; 1979, and Supplement 2 dated September 7,1979 for Three Mile Island Nuclear Station, Unit 1,. Docket No. 50-289.
The purpose of this report is to describe the results of piping system reanalysis and the resolution of nonconformances identified as a result of an inspection program to compare as-built conditions to design intent. As described in the interim report, issued May 2, 1980, an inspection program was implemented to verify that the type, location and orientation of piping, type and location of supports, pipe attachments, valve and valve operator orientation and weights, and -
pipe geometry were installed as input into the seismic analysis.
In conjunction with th,e inspection program, engineering evaluations were performed on piping supports *and systems with nonconformances to determine whether or not the as-built conditions meet design requirements. Supports which do not meet design requirements will be redesigned and construction modifications .

The following is a list of safety-related f 'smic Category I piping systems I consistent with the scope of inspections for IE Bulletin 79-14. The inspections of these systems or portions of systems are complete in accordance with the inspection procedure implemented for TMI Unit 1 (See Appendix II). Also listed for reference are system identifying codes.

TdR-TMI-194 Page 4 The flow diagrams included in Appendix IV identify by analysis identification number, (ME-No.), those portions of safety-related Seismic Category I systems which were inspected. Information from the original flow diagram has been eliminated to provide clarity.
The subdivision of systems into specific ME-No. was done to facilitate the computer modeling of the piping for analysis purposes. The boundaries of the analyses were telected on the basis of anchor points, or other physical limitations.
Appendix I provides a detailed list of piping isometrics, piping layouts drawings, and flow disgrams used for analysis input. This appendix is seperated into those systems analyzed and further subdivided into the actual analysis number.
The revision of drawings being used to perform inspections and verifications of r*,e analyses corresponded with the latest revision current on the date of the inspection or verification. When the document used to verify that analysis had been revised from that used in the original analysis, such revisions were reviewed to insure that they had been included in an updated analysis or did not affect it.
2.2 INSPECTION PROCEDURE The following is a summary of the procedure used by inspectors at the plant site
- in checking the geometry of piping and configuration of pipe support assemblies.
The inspection procedure is more fully described in Appendix II.
2.2.1 General A support assembly was defined as a rigid support, variable and constant support, or seismic restraint. The complete support, including ' support attachment or bracing steel was inspected. ,

TDR-TMI-194 Page 5 2.2.2 Inspection Tolerances 2.2.2.1 Pip support locations were inspected relative to tolerances related to pipe dia:eter. Refer to Appendix II. If tolerances were not met, actual locations were recorded.
2.2.2.2 Pipe geometry was inspected against a tolerance of one pipe diameter in any direction. If this tolerance was not met, the actual diatnsions were recorded.
2.2.2.3 Valve and valve operators were inspected against an axial and radial tolerance. Nonconformances were recorded.
2.2.2.4 Support / restraint structural attachment orientation in relation to the pipe attachment was inspected against a tolerance of plus or minus seven (7) degrees in any direction. Nonconformances were recorded.
2.2.1.5 Snubbers were inspected for size, orientation, and setting.
Nonconformances were recorded.
2.2.3 Inspection Elements 2.2.3.1 Support assemblies were inspected for the following:

i Assembly component size and quantity 2.2.3.2 Clearances were recorded at four 90* locations for wall penetrations, and pipe box restraints. This information was used to evaluate any possible adverse effects on thermal or seismic displacements.

2.3 SITE INSPECTION The following is a description of the site inspection program for
- piping systems and supports including methods of inspection and reporting on nonconformances.

2.3.1 Methods of Inspection The inspection procedure (see Appendix II) identifies the requirements for the inspection of Seismic Category I piping and pipe support assemblies.
Two principal documents used in the inspection were:
a ?iping Analysis Isometric Drawings
. These drawings were part of the design input for the original dynamic analyses of Seismic Category I systems. They define the pipe geometry, support locations and function, valve locations and orientations, pipe sizes and other elements affecting the analyses. ,
- b. Pipe Support Design Drawings I
These ar6 drawings of pipe supports, hangers and seismic restraints which show the type, location and configuration necessary to satisfy analytical requirements.
Additiotsi reference documents used are listed in Section 7.0 and
- include physical piping arrangement drawings, flow otagrams and vendor drawings. The inspectors worked in teams and divided the piping systems into work packages including an analysis isometric and related support assembly drawings. Nonconformances determined by comparison of 4

. recorded on inspection data sheets. Samples of the data sheets are included with Appendix II.
In those cases where thermal insulation prevented the inspection of '
~
critical support elements (such as pipe attachments) the insulation was -
removed for the inspection of 10% of the obstructed pipe supports for both normally accessible and inaccessible systems. Additionally, on the balance of the obstructed pipe supports, that portion of the support not covered by insulation was inspected.
2.3.2 Inaccessible Elements - Visual Inspections In accordance with the inspection procedure (Appendix II, Section 1.8),
and Bulle' tin requirements, any elements which were either inaccessible or were visually inspected in lieu of actual measurements are identified. Appendix V lists any elements in these categories along with reasvns for their inspection status and any safety consequences this status may have on their respective systems.
2.3.3 Reporting Nonconformances A unique number wa's assigned to each inspection report by the inspectors. For supports, the support number was used and for piping systest, ibe NE number was used. Piping nonconformances were reported on a Piping Inspection Report (PIR) form and transmitted to engineering for ev.luation. Support nonconformances were reported on a Support Inspection Eeport (SIR) form and submitted to engineering for evaluation.

TDR-TMI-194 Page 8 3.0 EVALUATION 3.1 Nonconformance Evalration - Piping Analyses Subsequent to field inspections, the PIR's were submitted to a piping analysis group for evaluation. PIR's were reviewed to evaluate whether the nonconformances recorded would necessitate reanal ' ,s. Reanalysis was not required if a nonconformance would not adversely affect pipe stress levels. New load sheets, however, were generated for all supports on these non-reanalyzed lines for review by engineering. This review considered any effects,of nonconformances on support capabilities. A summary of systems and nonconformances is listed in Appendix IIIA.
Valve manufacturers' drawings were used to verify that the valve weights and center of gravity used in the analysis were representative of the installed valve. Where manufacturers' drawings were unavailable, vendor catalogs or conservative estimates were used to define valve data. The line specification and original design were ,
reviewed to verify that the specified pipe wall thickness and material properties were input into the analysis. Any nonconformances identified were incorporated into a reanalysis.
3.2 Reasons for Reanalysis 3.2.1 Nonconformances The following is a list of 9 typical nonconformances which may have '
necessitated piping reanalysis:

Some reactor building piping was analyzed using response
- curves for internal floors in lieu of reactor building shell curve which was unavsilable at the time of the original analysis, 3.2.2 . Additional Considerations for Reanalysis The following is a list of additional factors which were considered in the reanalysis process:

Computer Program,' PIPDYN II, in an effort to reduce the conservatism in pipe support reactions, thereby minimizing amount of support rework. The decision to use this program, in certain cases, was made based upon original pipe stress levels and the number and types of nonconformances found.

Appendix IIIA indicates by use of the nonconfonsance code number, the 1
nonconformances which apply to a particular reanalysis. This Appendix also indicates the results of the analysis; supports added or supports
. deleted. Analysis results are summarized in Section 5.0.
3.3 Pipe Support Evaluation ,
All supports were submitted for engineering review. This review process was used to determine if an evaluation was required. A pipe support evaluation was performed for the fallowing reasons:
- a. Nonconformances discovered as a result of field inspections and/or 1 engineering review.
- b. Piping reanalysis indicated reaction forces higher than original analysis.
The objective of the evaluation was to determine whether or not a support met
. design requirements. Seismic Category I pipe supports were evalua,ted by engineering using the original design criteria. When a support was not 1
acceptable as installed, it was designated for redesign. Appendix IIIB tabulates by ME-Nos., the results of pipe support evaluations. This Appendix includes supports from formally analyzed lines and seismic spacing criteria supported lines.

TDR-TMI-194 Page 12 4.0 RESULTS 4.1 Pipe Support Redesign .
The redesign of existing supports or the design of new supports is intitiated by the definition of an RDR (Redesign Required). The RDR package contains all information compiled on the supports including field inspection reports, load sheets, support drawings, and evaluation calculations. The Support, Package cover 1 sheet indicates che components which failed to meet design requirements.
A design input document was prepared as a basis for redesign. This document contains the procedure for applying loads and also gives the allowable stress limits for all components used. The design of new supports is similar to the redesign of supports where additional members were added. Location and type of restraint is provided on a support drawing. The designer then checks other discipline drawings to find an' acceptable location for attachment. Where additional information is needed on accessibility or location, field personnel are consulted through a formal field request procedure. Engineering then
, finalizes the design using the most current loads.
An Engineering Change Memo (ECM) is issued in order to initiate the rework resulting from the need of additional, deleted or redesigned supports. Examples of redesign as implemented by an ECM are as follows:
l a. Add additional structural bracing to an exirting support .
( b. Change an existing tension rod support to a sway strut
- c. Add shear lugs to pipe to resist axial pipe loading
- d. Replacement of existing spring cans or snubbers.
l An evaluated support may have been identified for redesign, although the fix could actually be categorized as a maintenance ites. A Maintenance Inspection Report (MIR) would be implemented rather than an ECM. Examples of items placed on an MIR are as follows:

5.0 CONCLUSION
S The verification of safety related piping system seismic analysis is accomplished in the following four parts, as previously discussed:

- d. Support, Redesign The following provides a susuury of the results of each of the four parts completed to date:*
- a. Site Inspections Site inspection functions are complete. Of the 21 systems, approximately 2529 Seismic Category I supports and 54 Seismic Category III supports were inspected.
- b. Piping Reanalysis The PIR's received from the field were evaluatea to determine whether formal reanalysis was required. This resulted in 141 computer reanalyses and 64 original analyses reviewed by engineering and I accepted without formal reanalysea. Nonconformance effects on supports, however, are evaluated. The remaining ME(s) were accepted as found.

- c. Support Evaluation Supports were reviewed to determine if support evaluation was required as described in Section 3.3. The number supports reviewed and those l also evaluated after review total 2676. The disposition of these indicate 2012 supports acceptable and 664 supports needing redesign. 1 An additional 143 new supports are required as a result of reanalysis

RECOMMENDATIONS 6.1 Document Control This section describes the control procedures that have been implemented to '
reflect the "as-built" condition.
Interim drawings are prepared to depict a proposed support modification or -
addition as required. Once approved, these drawings are issued to construction for installation by an Engineering Change Memorandha (ECM). The ECM provides specific details referenced in specifications and construction procedures. The construction is initiated by release of a Work Authorization Notice (WAN).
Construction problems are submitted to Engineering by a Field Questionnaire (FQ).
After resolution of FQ's and completion of the repair, the pi Re support modification / addition is inpsected by QC for conformance with the drawings. Any i discrepancies between the installation and the drawing requirements are documented on an Incomplete Work List (IWL) and distributed to the responsible parties for resolution. The IWL items are reviewed for resolution and returned for correction in accordance with the drawing. Corrective action is initiated by the WAN.
Aft $er the IWL items are resolved, a copy of the ECM Interim Drawing is marked and stamped "ss installed", then submitted to engineering by a Design Change Notice (DCN). The "as installed" drawings are filed with the "as-built" records.

INSPECTION PROCEDURE 1.0 GENERAL PROCEDURE 1.1 Inspection of supports and piping geometry of Seismic Category I systems shall be as specified herein.
1.2 Piping supports to be inspected are fixed hangers and supports, j variable and constant-support hangers and supports, seismic restraints and hydraulic snubbers. They shall be termed supports for this procedure.
1.3 Piping geometry shall be inspected for arrangement and location of
~ supports and valves in comparison to the mechanical isometrics and piping arrangement drawings.
1.4 Nonconformances and significant deviation.: shall be evaluated within the requirements of the plant tech-specs to determine their effect, if any, on the ability of the system to function.
1.5 Deviations which decrease the function of load capacity of a suppc.t shall be indicated on an action ites or support inspection report.
An engineering evaluation shall be made to determine the support's capability and the systems ability to function. If the support's capability is determined to be insufficient and/or the system function is disabled, then the Manager THI-I Engineering or the TMI-1 Lead Mechanical Engineer shall be promptly notified in accordance with the requirements of IEB 79-14 including Supplement 2.
1.6 Damaged supports shall be indicated on an action item or support inspection report and sutaitted for Engineering Evaluation.
- 1.7 Where a support deviates from the drawing, sufficient and detailed information shall be provided (on a separate sheet if necessary) to allow an Engineering Evaluation and to allow drawing changes to be made.

1.11 Observe Radiation Work Permit (RWP) requirements for all l radiation / contamination areas.

2.1 PIPING INSPECTION REPORT

isometric. If a portion of a* system appears on more than one isometric, the isometric on which that system portion was inspected i
shall be referenced on the other isometric's systes evaluation data sheet.
i 2.2 PIPE SUPPORT ATTACHMENT 2.2.1 Location of pipe support attachment to pipe shall be inspected and compared to the piping isometric.

-20" to 36" f12" 2.2.3 Support attachment locations not within tolerance shall be recorded on the piping isometrics for actual dimensions. The support shall be identified on the PIPING INSPECTION REPORT.
2.2.4 Supports not shown on the piping isametrics shall be located on the drawings. A sketch showing function and attachment shall be attached to and referenced on the PIPING INSPECTION REPORT.
I 2.2.5 The piping shall be checked for unspecified interferences which limit the thermal movement of the pipe. These shall be located on the piping isometric and referenced on the PIPING INSPECTION REPORT.
2.3 VALVES 2.3.1 Valves shall be located within tolerances specified in Item 2.2.2 of dimension shown on piping isometric.
2.3.2 Valve operator rotetion from piping axis. shall be within 15* of that shown on drawing or to next bolt valve bonnet if flanged bonnet valve.
l 2.3.3 Valves or valve operators not within tolerance shall have actual dimensions / angular rotation recorded on the piping isometric and referenced on the PIPING INSPECTION REPORT.

TDR-TMI-194 APPENDIX II Page 4 of 10 2.4 SUPPORT ORIENTATION 2.4.1 Support structural attachment orientation with the pipe attachment shall be within 17' of specified orientation.
4 2.4.2 Beam brackets or the equivalent may be rotated 90' for rigid rod hanger supports.
2.4.3 Beam brackets used with sway braces shall be oriented so as not to hinder axial movement, unless otherwise specified on the drawing.
j 2.4.4 Restraints designed to act in pairs shall provide the same function
, as the designed supports.
2.5 PIPE GEOMETRY Pipe geometry shall be as shbwn on the isometric drawings. Where geometry deviates by more than one pipe diameter it shall be recorded on the isometric and referenced on the PIPING INSPECTION.

3.0 SUPPORT INSPECTION 3.1 SUPPORT INSPECTION REPORT 3.1.1 A SUPPORT INSPECTION REPORT shall be completed for each support inspected.
3.1.2 The support including attachment to plant structure shall be as shown on the support drawing. Deviations not allowed within this procedureshallberecordedontheSUPPORTINSPECTIONR$PORTand evaluated by Engineering.

. Where concrete expansion anchors are shown, the number and size only shall be recorded. Loone anchors shall be identified.
3.2.2 Supports attached by Unistrut shall have the number and size of bolts in Unistrut recorded.
3.2.3 Embedments shall have dimensions and location of support attachment as shown on the drawings. Deviations greater than 1" shall be recorded.
3.2.4 Base plates shall be checked for the following (record deviations greater than 10 percent of measure quantity):
- 1. . Plate thickness, dimensions, orientation
- 2. Anchor location, size, and quantity
- 3. Edge distance from center of anchor to plate edge (if less than 2 X diameter of bolt, record) l
- 4. Location of support attachment.
3.2.5 Anchors for other supports within 8 in. center-to-center shall be shown on support drawing.
3.2.6 Edge distance of anchor center-line to edge of concrete shall be shown if less than 6".
3.2.7 Floor and wall penetrations shall have clearance as specified on drawing. Check at four locations 90 apart. If no clearances specified, measure and record.

3.3.1 3.3.2 Rigid Rod Hangers .
- 1. Single rigid rod hangers may be interthanged with double-rod trapeze hangers provided new support is adequate to carry load.
- 2. Double-rod trapeze hangers may be interchanged with single rigid rod hargers provided single rod diameter is 1-1/2 times or greater than double-rod diameter.
3.3.3 Spring Hangers l

- 1. Types "A", "B" AND "C" spring hangers may be interchanged.

- 2. The elevation of Type "A", "B" OR "C" spring may be different.
3.3.4 Hydraulic Snubbers -
1 A large bore snubber may be substituted for a smaller bore snubber.
- 2. The fluid reservoir may be reversed from the drawing provided l the vent hole is on the 'high side of the snubber.
l 3. Yurcaded end of the snubber shall be on the support structural attatnment (building) side.
( 4. Snubber shall be F 1/4" from full extension or full compression.

3.5.2 All components shall be installed. Missing components shall be ,.
identified.
3.5.3 Two steel plates may be substituted for a beam bracket. Record dimensions of plates.
3.5.4 Attachment plates with forged clevises may be interchanged with beam brackets with welded eye, rods if equal to or heavier size than shown on drawing.
3.5.5 Weldless eyenuts with threaded rod may be interchanged with welded eyeroda if equal to or heavier than shown on drawing.
3.5.6 Field fabricated pipe insulation saddles may be substituted for manufactured pipe insulation saddles. Record dimensions.

3.8.3 THREAD ENGAGEMENT

- 2. Clevis / turnbuckles shall have full thread engagement (one full thread exposed).
- 3. Less than full thread engagement shall be recorded. Indicate -
1 diameter and depth not engaged in nut / clevis / turnbuckle.
3.9 MAINTENANCE ITEM REPORT 3.9.1 Items found during inspection which do not affect the function or load capacity of a support shall be identified on a MAINTENANCE
. ITEM REPORT for action by the Plant Staff as necessary.
3.9.2 Examples are:
- 1. Loose nuts with full thread engagement (except on tight U-bolts). .
- 2. Loose pipe clamps.
- 3. Loose bolts when used in shear only.
