This refers to the inspection conducted by Mr. W. E. Foster of this office on March 23-27, 1981, of your facility at Englewood, Colorado, associated with the fabrication of optical isclators and to the discussions of our findings with you e.nd munbers of your staff at the conclusion of the inspection.
l This inspection was made as a result of the identification at Catawba Units 1 and 2 and McGuire Units 1 and 2 of improper operation of digital optical isolators I
when driving loads that surge in excess of one ampere. During the inspe:: tion.
nunerous instances were identified and documented in the enclosed inspection report where you failed to comply with NRC requirements. The specific findings, canplete with reference to the applicable requirements, are summarized in the enclosed Notice of Violation and Notice of Nonconformance.
The main purpose of the inspection was to establish whether quality assurance program controls relative to the manufacture of optical isolators were consistent with applicable codes, contractual and regulatory requirements.
It is apparent from the results of this inspection that significant deficiencies exist in regard to both the quality assurance organization and the implementation of your quality assurance program.
What concerns us greatly is that tne scope and nature of the findings are such, that they would appear to indicate insufficient recognition and attention are being accorded to the quality assurance function by the responsible officers of your company.
f The Notice of Violation is sent to you pursuant to the provisions of 10 CFR Part 2.201.
You are required to submit to this office within 25 Jays of the date of this letter, a written statement containing (1) a description of steps that have been or will be taken to correct the itens, (2) a description of steps that have been or will be taken to prevent recurrence, and (3),he dates your corrective actions and preventive measures were or will be completed.

In accordance with Section 2.790 of the Commission's " Rules of Practice,"
Part 2, Title 10, Code of Federal Regulations, a copy of this letter with enclosures and your reply will be placed in the Commission's Public Document Room.
If this report contains any information that you believe to be proprie-tary, it is necessary that you make a written application to this office within 25 days of the date of this letter to withhold such information from public disclosure. - The application must include a full statement of the reasons why it is claimed that the information is proprietary. The application should be prepared so that any proprietary information identified is contained in an enclosure to the application, since tha application without the enclosure will also be placed in the Public Document Room.
If we do'not hear from you in this regard within the specified period, the report will be placed in the Public Document Room.
