( PENDIX A NOTICE OF VIOLATION South Carolina Electric and Gas Company Docket No. 50-395 V. C. Summer License No. CPPR-94 As a result of the inspection conducted on July 13-17, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violation was identified.
10 CFR 50, Appendix B, Criterion V as implemented by V. C. Summer FSAR section 17.2.5 requires in part that activi'.ies affec$ ng qualit_ shall be prescribed by procedures and shall be accomplished in accordance with those procedures. Westinghouse procedure ISI-205, Rev. 2, figure 7 requires a minimum scan length of 4 inches when a full node 45 degree examination is performed on material ranging in thickness from 1 f rch to 1.2 inches. In addition,. paragraph 3.7 of ISI-205 requires the surface finish on the calibration block to be representative of the examination area and paragraph 2.4 requires the area contacted by the transducer to be checked to ensure that foreign matter that could impair the free movement of the transducer or effect the examination be rectified prior to conducting the examination.
Contrary to the above, on July 15, 1981, activities affecting quality were observed that would have prevented accomplishment of preeiously reported completed ultrasonic inspections in that Feedwater System welds #121 on Isometric CGE-2-2103 and #4 on Isometric CGE-2-2304 were observed painted within the required UT scan area.
The licensee's calibration blocks were not representative of the examination area in that they were.not painted to compensate for signal losses result:ng from sound that was attenuated by the paint. In addition, the surface condition at the interface between the non painted area and the painted area precluded the full inspection of the weld and base metal area of interest.
This is a Severity Level V Violation (Supplement II.E).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of t'is Notice, a written statement or explanation in reply, including: (1) admissir;1 or denial of the alleged viola-tion; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
