Appendix A NOTICE OF VIOLATION Toledo Edieon Company Docket No. 50-346 As a.esult of the inspection conducted on July 1-31, 1981, 'nd in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),
the following violation was identified:
Technical Specification 4.0.5 states: Surveillance testing on ASME Class 1, 2 and 3 components shall be conducted in accordance with Section XI of the ASME Boiler and Pressure Vessel Code 1974 Edition, Addenda through summer 1975.
The ASME Boiler and Pressure Vessel Code 1974 Edition, Addenda through summer 1975 requires that valves which operate in the course of plant operation need not be additionally exercised, provided that the observa-tions otherwise required for testing are made and recorded during such operdtion.
Davis-Besse Surveillance Procedure ST 50'9.11, requires that each time one of the valves listed in the procedure is operated, it shall be recorded.
Valves D0-81 and D0-82 are listed in Procedure ST 5099.11 Contrary to the above, on July 7, 1981, the inspector observed the operation of Valves D0-81 and DO-82 by plant operators and no log entry vas made to record the valve operation.
This is a Severity Level VI violation (Supplement I).
The inspection showed that action had been taken to correct the identified item (s) of noncompliance and to prevent recurrence. Consequently, no reply to this (these) item (s) of noncompliance is required and we have no further questions regarding this matter.
