s General Electric Company Docket No. 99900003/81-02 NOTICE CF NONCONFORMANCE Based on the results of an NRC inspection conducted on July 20-24, 1381, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
Section 5, Instructions, Procedures and Drawings, of the General Electric Topical Report, NED0-11209-04A, states in part, " Activities affecting quality,... are delineated, accomplished, and controlled by such documents as policies, pro-cedures...."
I A nonconfomance with these requirements is as follows:
Quality Assurance Inspecti n Instruction 6.2.5.1, (GI, U) 02 Pellet Sampling and Release, Revision 5, paragraph 12, requires the computer system to calculate 4
acceptance to the total impurity specification limit for each lct of pellets.
The total impurities are based on three samples, one for fluorine, a second for chlorine, carbon, and nitrogen and a third for metal irapurities (19 elements).
Contrary to the above, it was found that the samples used by the computer to accept specific pellet lots did not always represent the same lot of pellets that was accepted.
Three Im:s of gadolinium oxide fuel teere reviewed and it was found that the flourine samples had been drawn from a different pellet lot than the metal impurity samples for two of the lots.
The remaining lot had the chlorine, carbon, and nitrogen sample drawn for a different pellet lot than the metal impurity sample.
