General Electric 4.16 KV Switchgear Containing Internal Wiring Which Violates Class JE/Non-Class 1E Separation Requirements

Dear Mr. Seyfrit:
On September 18, 1981, Arizona Public Service, PVNGS 1, 2 and 3, informed Mr. B. H. Faulkenberry, Chief Reactor Construction and Engineering Support Branch, of the subject deficiency.
Subsequent evaluation by the Bechtel Review Committee concludes that the condition is also reportable under 10 CFR Part 21. The Arizona Public Service report contains all the information required by Part 21 with the exception of the information required by 21.21(b)(3) VI as it is unknown to Bechtel.
General Electric will be advised by letter that the component as applied by Bechtel contains a substantial safety hazard defect and will be requested to review the information supplied by Bechtel and determine its responsibility for compliance to Part 21, Section 21.21(b)(3) VI.
Other Bechtel Divisions were advised of the defect by copies of appropriate reports and correspondence. Should it be determined that basic components with this defect are in use, have been supplied to or are being supplied for one or more other Bechtel projects, the affected Division will report the condition according to the Regulation.
