401. 2 8 b M APPENDIX A NOTICE OF VIOLATION Carolina Power & Light Company Docket Nos. 50-324 & 50-325 Brunswick 1 & 2 License Mos, DPR-62 & DPR-71 As a result of the inspection conducted on June 15 - July 15,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (Oct ber 7,1930),
the following violations were identified.
A.
Technical Specification 6.8.1 requires that writteri procedures shcIl be established, implemented and maintained. Operating procedure 01-5-A, Status of Annunciators in Alarm, requires annunciators in alarm to be recorded in a log.
Contrary to the above, on July 6, 1981, alarming annunciators on control room alarm panels 2-A-1 throu. h 2-A-7, were not listed in the log. Unit 1 and Unit 2 control operators were not maintaining the log as required.
This is a Severity Level V Violation (Supplement I.E.).
B.
Technical Specification 3.0.4 requires that entry into an Operational Condition or other specified applicability state not be made unless the conditions of the Limittag Condition for Operation are met without reliance on provisions contained in the Action Statements. Technical Specification 3.5.3.2, requires that two independent Low Pressure Coolant Injection (LPCI) subsystems of tne residual heat removal system shall be operable with each subsystem cor.iptised of:

Two pumps j

An operable flow path capable of taking suction from the suppression l
pool and transferring the water to the reactor pressure vessel.
j Contrary to the above, as a result of an operator error, the reactor mode l
switch was taken out of the refueling mode, placed in startup, and control rod withdrawal comenced with the A-loop RHR torus suction valve (F020A) shut.
This is a Severity Level V Violation (Supplement I.E.), applicable to Unit 2.
