Oyster Creek Nuclear Generating Station Docket No. 50-219 Inspection Report No. 50-219/81-12 In accordance with 10 CFR 2.201, the enclosure presents our response to the Notice of Violation involving an unlocked door at the entry to a high radiation area.

In this response, only Item B of the Notice of Violation has been addressed since your letter of transmittal dated July 31, 1981, states that no response to Item A is required.

'Ibe Notice of Violation ocntained in IE Inspection Report 81-12 states in part:

Technical Specification 6.13.2 states, " Specification 6.13.1 shall also apply to each high rcdiation area in which the intensity of radiation is greater than 1000 mrem /hr. In addition, locked doors shall be provided to prevent unauthorized entry into such areas and the keys shall be maintained under the admmistrative control of operaticns and/or radiation protection supervision on duty".

Contrary to the above, at about 2:00 p.m. on June 16, 1981, the door to the reactor building southwest corner room, an area in which the intensity of radiation is greater than 1000 mram per hour, was left unlocked with no means of preventing unauthorized entry.

'Ihis is a Severity Invel IV Violation (Suppleent IV.D.).

Jersey Central Power & Light Ocupany has determined that this violation is correct as stated. 'Ihe following presents our response to the violation.

Corrective steps which have been taken and results achieved.

'Ibe door in question ms prcmptly repaired and restored to the locked position. A critique of the incident revealed that @n entry, the individual who left the door unlocked noticed a mechanical problem which caused difficulty in closing the door; 4

however, this individual did noc r==har taking action to assure the door was properly locked while exiting. Disciplinary action was taken to reinforce the requir m ents for maintaining doors to high radiation areas in the locked positicr 2.

Cbrrective actions which will be taken to avoid further violations.

'Ihe procedure governing surveillance of high radiation area doors has been revised to require more frequent inspections of these areas. 'Ihis, in contniction with the disciplinary action taken above, will serve to prevent a recurrence of this violation.
