Appendix A NOTICE OF VIOLATION Nuclear Diagnosis, Incorporated License No. 24-17561-01 Springfield Osteopathic Clinic As a result of the aspectio2 conducted on August 27, 1981, and in accord-ance with the Interin Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:

License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated May 16, 1977 and letters dated July 19, i

1977, November 25, 1978, May 21 and December 3 and 14, 1979. Letter dated July 19, 1977, Paragraph No. 7, states that specific areas of the laboratory will be surveyed after preparation of the pati-nt doses and prior to transportation.

Contrary to the above requirement, as of the date of this inspection, this requirement was not met.

Specifically, daily area surveys of tae hot laboratory were not performed.

10 CFR 35.14(e)(1)(i) requires that sealed sources used as calibration or reference sources be tested for leakage and/or contamination at intervals not to exceed six months.

10 CFR 35.14(e)(2) requires that records of leak test results shall be kept in units of microcuries and maintained for inspection by the Commission.

Contrary to this requirement, a 193 microcurie cesium-137 sealed i

source, Sa-ial No. 319-069-10, was not leaked tested within the last twelve months.

This is a Severity Level V violation (Supplement VII).

Pursuant to the provisiens of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:

(1) cor-rective action taken,nd the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or 4
