3 SPECIAL REPORT During the special low power test program for North Anna Unit 2, authorized by the Nuclear Regulatory Commission on July 3,

1980, PORV PCV-2455C was challenged a total of four times.

As described in our " Natural Circulation Test Report" which was forwarded to you on July 22, 1980, via letter Serial No. 648, the first three challenges occurred on July 5, 1980 during the performance of Special Test 2-ST-8, " Natural Circulation Verification."

The fourth challenge occurred on July 6, 1980 during the performance of Special Test 2-ST-9, " Natural Circulation with Simulated Loss of Power Conditions."

A deviation report was initiated for these challenges.

These challenges occurred at a time when applicable reporting requirements had not been established.

In response to NUREG 0660, TMI Action Plan Item II.K.3.3, in a letter Serial No. 560, dated June 30, 1980, we stated that a program was being established to identify, record, and report failures of or challenges to power-operated relief valves or safety valves.

In a letter dated July 22, 1980, Serial No. 645 we proposed changes to the North Anna Unit i

2 Technical Specifications to provide annual reporting requirements for such an event. When NRC issued the Full Power License and Technical Specifications on August 21, 1980, challenges to PORVs and/or Safety Valves were included as part of the Monthly Operating Report requirements.

In that the reporting requirements were not firm at the time of the events, the deviation report identifying the events was classified as an annual l

report.

Following examination of the matter we have determined that it would not be appropriate to include this in a monthly report but that it should rather be reported as a special report. As stated above the NRC was informed about the challenges in a timely manner; however, this special report should serve to close this matter.
