Dear Sir:
South Texas Project Units 1 & 2 Docket Nos. STN 50-498, STN 50-499 Interim Report on the Containment Gas Analysis System Overpressurization On April 24, 1980, Comsi,, Inc. reported to your ofrice pursuant to 10 CFR 21 a possible defec: in containment gas analysis systems manu-factured by their Delphi Systems Division.
In their letter, they identi-

Mr. Tom Jordan of HL&P discussed this matter with Mr.
H. S. Phillips of your office on May 16, 1980.
l During a subsequent conversation with Mr. Phillips on June 4,1980, l
he stated that Houston Lighting and Power Company would be required to j
furnish your office with a written report pursuant to 10 CFR 50.55(e) if l
the component identified above had been delivered to the STP site.
Mr.
Phillips confinned this interpretation with Mr. Bill Crossman of Region IV.

On June 5,1980, HL&P confinned that the eqaipment had been delivered to the STP site and informed Mr. Phillips via telephone that same day. Mr.
Phillips requested an interim written report on the matter.
As stated in the Comsip letter of April 24, 1980, repair kits will be furnished to all ;icenseas as soon as possible.
To date, HL&P has not re-ceived a repair kit; but, efforts are being made to expedite this repair.
Mr. H. S. Phillips will be notified when the repairs have been completed and a Final Report will be submitted to your office.

Notification of Defect as follows:
l I was notified yesterday, 2 April 1980, that while in-house l
testing war bei.ig conducted, the following failure occured.
The contents of the report made to me by one of our Test Technicians follows:
Subject:
8^-637 "K" Pump Malfunction 1
During the testing of 80-637, one of the panel outlet valves was closed while the panel was shut-
'down.
The pump was then started with the valve

still closed.
The followins occurred;

- 3) The pecuuure buildup caused the connecting rods to separate from the crank shaft causing a tremendous strain on the diaphragms.
It is conceivable that these diaphra5ms could have ruptured, therehy breaching t'he integrity of the system.
The panel valves arc one cause for such an event, but also the "It" stamp containment isolation valves, which are cometimco automatically ~
closed, could cause the same malfunction.
To remedy the prob 1cm, we could install a check valve acrocc the inlet and outlet of the pump.
In the event of any valves being closed in the system, the check valve would prevent any damaging build-up of pressure."
s The above report make.. clear that either of the following two safety hc7.crds exist:
1) up to one litre of containment gas could be released into the outside atmosphere, should the f ailure as s
de"ined above occur; and/or 2) if the input / outlet valve is closed while the pump is in operation or if the containment isolation valve inside the containment is closed, there is a riu'A in damaging the system, thereby rendering the system inoperable.

Corre?tive Action to be Tcken: We arc in the pr occu u of put ting a by-pass on the discharge of the pump to prevent deadheading, thereby preventing any damage. This corrective action will be it.troduced on the Cleveland Kuclear Plant and will be added on all future jobs. Notification of Tacilities/ Purchasers: Notification will be'made to the above facilities immediately of the defect and the corrective actions being taken and that kit will provided them ou voon as possible which will totelly 4 correct any possibility of failure / damage. for further information, the work necessary to comnlete this corrective action is very minor in nature and can be easily ( accompliched on uite by a quellfled piperitter.
