m Appendix A NOTICE OF VIOLATION Cincinnati Gas and Electric Company Docket No. 50-358 As a result of the inspection conducted on June 29, July 1, and July 20-21, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (Oc+.ober 7, 1980), the following violation was identified:
10 CFR 50, Appendix B, Criterion IX states in part that, " Measures shall be established to assure that special processes, including... nondestructive testing, are controlled and accomplished by qualified personnel using qualified procedures in accordance with applicable codes, standards, specifications, critcria, and other special requirements."
Wm. H. Zimmer Nuc1 car Power Station FSAR, Chapter 17, Paragraph 17.1.9.2 states "Special processes are accomplished and controlled by qualified personnel using qualified procedures in accordance with applicable coder, standards, specifications, criteria, or other special requirements."
Chapter 3, Section 3.2 of the FSAR classifies systems according to the safety function they perform and specifies the following applicable Code for the piping in question.
American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel Code,Section III, 1971 Edition, Sammer 1973 Addenda, Appendix IX, Paragraph IX-3334.4 requires that the shim thickness shall be selected so that the total thickness being radiographed under the penetrameter is the same as the total weld thickness and paragraph IX-3334.5(b) states in part "... when the source is placed on the axis of the object and the complete circumference is radiographed with a single axposure, at least three uniformly spaced penetrameters shall be employed."
Contrary to the above, the shim thickness was not selected so the total thickness being radiographed under the penetrameter was the saie as the-total weld thickness for one weld.
In addition, when the source was placed on the axis of the object and the complete circumference radiographed with a single exposure, there was not at least thrae uniformly spaced penetrameters for another weld.
This is a Severity Level V violation (Supplement II).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
