I Appendix A NOTICE OF VIOLATION Swedish Covenant liospital License No. 12-02497-01 License No. 12-02497-02 As a result of the inspection conducted on August 20, 1981, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),
the following violations were identified:
License No. 12-02497-01 1.
License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated April 23, 1978, and letter dated May 8, 1978.
Item No. 7b. of the above referenced application states the Medical Isotopes Committee will meet quarterly.
Contrary to the above requirement, it was determined through statements by licensee representatives and the NRC inspectors review of records that this condition is not being met.
Specificall3, the last Medical Isotopes Committee meeting was held on November 7, 1979.
This is a Severity Level V violation (Supplement VII).

License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated April 23, 1978, and letter dated May 8, 1978. The above referenced application states, the procedures described in Appendix D of the draft " Guide for Preparation of Applications for Medical Programs" will be followed. Appendix D, Section 2 requires that your dose calibrator be checked and recorded quarterly for linearity and annual accuracy, using eeveral radionuclides such as cesium-137, barium-133 and cobalt-57.
Contrary to the above requirement, it was determined through statements by licensee representatives and the NRC inspectors review of records that this condition is not being met.
Specifically, a licensee representative stated that since the inception of this license, the only linearity check was performed in April 1931, and the record of the results were not kept.
In addition, accuracy checks are being performed using only cesium-137.

License Condition No. 16 requires a set of written emergency instructions be posted at the teletherapy machine control. These instructions shall inform the machine operator of the procedure to be followed should he be unable to turn the machine's primary beam of radiation "of f" with the controls outside the treatment room.
Contrary to the above requirement, on the day of this inspection, the teletherapy control console was not posted as required.
This is a Severity Level VI violation (Supplement VII).

10 CFR 20,203(c) and (e)(2) requi res each entrance or access point to a high radiation area be conspicuously posted with a sign or signs bearing the radiation caution symbol and the words " Caution-High Radiation Area" and " Caution-Radioactive Material."
Contrary to the above requirement, on the day of this inspection, the entrance door to the teletherapy room was not properly posted.
This is a Severity Level VI violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including fr r each item of noncompliance:
(1) cor-rective action taken and the results chieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or a f firmation.
Consideration may be given to extending your response time for good cause shown.
