APPENDIX A NOTICE OF VIOLATION Louisiana Power and Light Company Cocket No. 50-382 CPPR-103
. Based on the results of an NRC inspection conducted during the period of June 16 - July 15,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following siolations were identified:

Criterion V of 10 CFR bO, Appendix B, states that activities affecting quality shall be prescribed by instructions, orocedures or drawings of a type appropriate to the circumstances and tnat these activities shall be accomplished in accordance with these instructions, procedures or drawings.
The Waterford-3 FSAR (17.2.6.2) and Quality Assurance Manual-Preoperational and Operational Phase-requires that obsolete or superceded documents shall be controlled by approved written procedures to prevent inadvertent use.
Contrary to the above, on July 13, 1981 the NRC inspector observed that QP-1-002, Station QC Review of System Release and Turnover Packcges, was not controlled in that no approved written procedures existed that described how this obsolete procedure was to be controlled and it was available for use in the Plant Operating Manual for over three months after it was deleted on april 3, 1981.
This is a Severity Level VI Violation.

Criterion V of 10 CFR 50, Appendix B, states that activities affecting quality shall be prescribed by instructions, procedures or drawings of a type appropriate to the circumstances and that these activities shall be accomplished in accordance with these instructions, procedures or drawings.
Startup administrative procedure, SAP-10, Temporary System Modification, in section 4.4.4 requires that temporary system modification tags have written on them, (1) date the modification was performed, (2) the name of the person making the modification,and (3) the modification description.
Contrary to the above, on June 30, 1981 the NRC inspectors observed that temporary modification tags 50A3, items 12 and 32, located in cabinets 31 and 40 respectively on the -15.5 foot level of the Reactor Auxiliary Building (RAB) did not include the date the modification was performed, the name of the person making the modification or the modification description.
This is a Severity Level V Violation.
