Appendix A NOTICE OF VIOLATION Audrain Medical Center License No. 24-15122-01 As a result of the inspection conducted on July 14, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:

1.

10 CFR 20.201(b) requires that such surveys be conducted as may be necessary for you to comply with all sections of Part 20.

Contrary to this requirement, your failed to make such surveys as were necessary to ensure compliance with 10 CFR 20.105(b), " Permissible levels of radiation in unrestricted area."

Specifically, you failed to make an adequate survey of the unrestricted area between the Nuclear Medicine Department and Radiology Room 4.

This is a Severity Level V violation (Supplement IV).

License Condition No. 14 requires that licensed material be possessed and used in accordance with statements, representations, and procedures con-tained in applications dated April 18, 1977, and March 3, 1980; and letter i

dated May 3, 1977.

The license application dated April 18, 1977, states that daily G-M surveys will be performed in the areas used for generator elution, radiopharmaceut-ical preparation, and patient dose preparation.

Contrary to this requirement, daily G-M surveys were not performed in the areas used for generator elution, radiopharmaceutical preparation, and patient dose preparation since the date of license issuance.

This is a Severity Level V violation (Supplement VII).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:

(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncom.pliance; and (3) the date when full compliance will be achieved.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.

Consideration may be given to extending your response time for good cause shown.
