APPENDIX A NOTICE OF VIOLATION Baltimore Gas and Electric Company Calvert Cliffs Nuclear Power Plant, Units 1 and 2 License Nos. DPR-53 Docket Nos. 50-317 DPR-69 50-31) i As a result of an inspection conducted on May 18-22, 1981 and in accordance with Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:

10 CFR 50, Appendix B, Criterion XII states, in part, " Measures shall be established to assure that... gauges land / instrument'..used in activities affecting quality are properly controlled, calibrated, and adjusted at l

specified periods to maintain accuracy within necessary limits."

Baltimore Gas and Electric Company Quality Assurance Manual for Nuclear 1

Power Plants, QA Policy-1, Quality Assurance Policy for the Calvert Cliffs 4

Nuclear Power Plant, states, in part, "...The quality assurance program was developed to meet the requirements of... Regulatory Guide 1.33-QA Program Requirements (Operation, Rev. 2, 2/78)...".

Regulatory Guide 1.33, Quality Assurance Program Requirements (Operation),

i Revision 2, February, 1978, Appendix A, Paragraph 8.1, states, in part,

" Procedures of a type appropriate to the circumstances should be provided to ensure that... gauges land / instruments are properly... calibrated and j

adjusted at specified periods to maintain accuracy.

Specific examples of such equipment to be calibrated and tested are readout instruments...".

Contrary to the above, the following are examples of installed test gauges

not being calibrated, which were used to verify the operability of safety related pumps per Procedure STP 0-73-1, " Engineered Safety Features Equipment Performance Test", as required by Technical Specifications and Inservice Inspection programs:

Salt Water Pump No. 11 Discharge Pressure.

Service Water Pump No. 11 Discharge Pressure.

HPSI Pump No. 11 Suction Presture Boric Acid Pump Discharge Pressure.

Further, based on discussions witF /our onsite representatives, that although the examples listed above were for Unit No. 1, this violation is also applicable to Unit No. 2.

This is a Severity Level V Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Baltimore Gas and Electric Company is hereby required to submit to this office, within thirty days of the date of this notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the.results achieved; (2) corrective steps which will be taken.to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is shown, consideration will be given to extending your response time.

The responses directed by this Notice are not subject to clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
